About this file

This document is a Cost Proposal form for Request for Proposal (RFP) 25-10325 issued by the California Department of Public Health (CDPH), specifically for a training contract related to asthma education and home health practices. The proposal covers seven distinct training modules, each to be delivered six times per fiscal year across three consecutive fiscal years (FY 25/26, FY 26/27, and FY 27/28). The modules cover topics including the scope of asthma, triggers and environmental control, medications, medication delivery devices, assessment and monitoring, home visiting best practices for community health workers, and conducting in-home asthma trigger assessments.

The contract has a total allocated budget of $300,000, with any proposals exceeding this amount being deemed non-responsive and ineligible for award. The rates proposed must be fully burdened, encompassing all associated costs such as operating expenses, labor, equipment, supplies, overhead, inflation adjustments, profit margin, taxes, and shipping. The agreement is structured as a deliverables-based contract, meaning payment is contingent upon successful completion of specified training modules. Bidders are required to complete and sign the cost proposal, with failure to do so potentially resulting in the proposal being classified as non-responsive.
