About this file

This document is a Request for Proposal (RFP) issued by the State of Michigan for dental equipment, supplies, maintenance, and repair services to support the Michigan Department of Health and Human Services (MDHHS) psychiatric hospitals and the Michigan Department of Corrections (MDOC) correctional facilities. The purpose of this RFP is to establish a statewide contract to provide preventative maintenance and repair services for dental equipment at these facilities. The contract will initially serve MDOC and MDHHS, with the State reserving the right to modify the contract to meet the needs of other State agencies and MiDEAL members. The successful contractor must work with program managers to establish maintenance schedules within 60 days of contract execution and provide quarterly reports on preventative maintenance and repair services performed.

Pricing is firm for the entire length of the contract, with provisions for price adjustments based on changes in the contractor's actual costs. The State requires the contractor to identify energy efficient, bio-based, or environmentally friendly products, disclose any hazardous chemicals or mercury content, and confirm that products do not contain brominated flame retardants or perfluoroalkyl and polyfluoroalkyl substances. The contract includes service level agreements with credits assessed for non-compliance, such as failure to complete preventative maintenance or repair services within the specified timeframes.
