About this file

This is a questionnaire (Appendix A, Part 1) for RFP No. 21-057/DP soliciting long-term disability (LTD) and short-term disability (STD) group insurance coverage for Palm Beach County, Florida, and its affiliated entities including the Board, PALM TRAN, and the Palm Beach County Supervisor of Elections. The RFP requests proposals for both LTD and STD insurance products with comprehensive administrative support. Respondents must provide detailed information regarding their company history in offering and administering disability coverage, current national and Florida enrollment figures, financial ratings, proposed benefit structures, and service capabilities. The questionnaire requires submission of sample policies, enrollment materials, certificates of coverage, and financial statements, along with identification of assigned account executives and service personnel. Respondents must confirm their ability to support multiple entities with overlapping open enrollment periods, provide electronic enrollment capabilities, accommodate evidence of insurability processes, and deliver customized enrollment materials at no additional charge.

The questionnaire does not specify pricing information, contract award dates, or contract term details within this document. However, it establishes critical service requirements including toll-free customer support, online claims systems with employer access, self-billing payment options, and a preferred 45-day grace period for premium payments. Proposers must address specific administrative accommodations such as waiving actively-at-work provisions during initial enrollment, handling sick time usage before disability benefits, addressing donation of vacation leave without offsetting benefits, and seamlessly transitioning claims from STD to LTD without additional evidence of insurability requirements. The questionnaire indicates the County currently uses electronic reporting processes for Evidence of Insurability and seeks vendors capable of maintaining this capability while accommodating alternative paper processes for the Supervisor of Elections. Respondents must also provide detailed rehabilitation provisions, Social Security filing assistance, grievance and appeals procedures, and fraud prevention strategies.
