About this file
This Addendum #1 to Request for Proposal #202601, issued by Kenosha County Department of Human Services on April 29, 2026, provides clarifications to proposers regarding Physical Therapy, Occupational Therapy, and Speech & Language Pathology services for Brookside Care Center and Willow Brook. The RFP seeks comprehensive therapy and management services for approximately 178 active residents across two facilities, with monthly service volumes of approximately 877 occupational therapy units, 1,052 physical therapy units, and 84 speech therapy units. The proposal response deadline is May 7, 2026 by 10:00 AM. The county expects to retain current therapy staff, which consists of five occupational therapists, five occupational therapy assistants, four physical therapists, six physical therapy assistant, one speech-language pathologist, one director with physical therapy discipline, and one coordinator—all currently employed as contracted staff with no open positions. The contract will operate on a three-year initial term through December 31, 2030, with options for additional one-year renewals subject to mutual agreement.
The county budgeted approximately $820,000 for therapy expenses in the upcoming year and will allocate 20 points of the 100-point scoring rubric to budget approach and cost-effectiveness. Pricing methodology shall be per-minute for Medicare Part A, Medicare Advantage PDPM plans, and HMO/MCO coverage, with percentage of fee schedule per CPT code for Medicare Part B and Medicare Part B replacement plans. The therapy provider will not serve as biller of record; instead, the facility will bill for Willow Brook Assisted Living residents and Brookside will bill using its existing NPI. Brookside Care Center will provide and maintain all therapeutic equipment including diathermy machines, NU Step equipment, treadmills, parallel bars, and treatment tables. The county utilizes MatrixCare as its electronic documentation and care planning system, Net Health as the therapy management software, and requires proposals to include financial statements (audited or unaudited), key personnel resumes, references from public or private sector organizations, and documentation demonstrating cost-effectiveness with not-to-exceed percentage increases for three successive years. State registration for therapy services is generally required at bid submission but will be accepted immediately following award.
