About this file
This document is a clarifying questions response issued by CareOregon, a 501(c)(3) nonprofit health plan in Oregon, for Request for Proposals #2025-06 regarding End Stage Renal Disease (ESRD) Population Risk-Based Management Services. CareOregon seeks a risk-bearing entity to provide comprehensive population health management services for approximately 200-250 ESRD members enrolled in CareOregon Advantage, a Dual Eligible Special Needs Plan (D-SNP) Medicare Advantage plan. The project includes designing kidney care programming, ensuring members receive all entitled benefits, and implementing proactive population health management for members with early-stage chronic kidney disease through ESRD. The RFP response deadline is January 6, 2026, at 2:00 PM Pacific Time, with clarifying questions due by December 22, 2025. The contract will commence in February 2026 for an initial three-year period through December 31, 2028, with options for two additional one-year renewals.
The fee structure allows flexibility with multiple pricing options available to proposers: risk-bearing arrangements, fixed fees, time and materials pricing with a not-to-exceed price, or contingency fees based on a percentage of total savings from data mining services. Proposers may also offer additional fee arrangements beyond these specified options. The RFP evaluation criteria allocate points across four areas: General Background and Qualifications (0-30 points), Scope of Work (0-35 points), Equity, Diversity, and Inclusion Efforts (0-10 points), and Fees (0-25 points), totaling 100 possible points. Eligible contractors must be risk-bearing entities capable of assuming medical expenses and demonstrating substantial experience with Medicaid, Medicare Advantage, CKD, and ESRD populations. Selected contractors must comply with HIPAA regulations and CareOregon's Data Security Requirements and maintain comprehensive insurance coverage including Commercial General Liability, Professional Liability, and Cyber Liability policies.

REQUEST FOR PROPOSALS #2025-06
ESRD Population Risk-Based Management Services
RESPONSE TO CLARIFYING QUESTIONS #2
December 18, 2025
Note that these are questions submitted by interested firms to the above referenced solicitation.
The below answers are for clarification purposes only and in no way alter or amend the RFP as published.
1. Do you have estimates on how many CKD patients would be eligible for this program? I believe in the RFP document it mentions roughly 200-250 ESKD patients only.
Answer: Correct. RFP Section, Project Background, states that “CareOregon operates a
Dual Eligible Special Needs Plan (“D-SNP”) Medicare Advantage plan (CareOregon
Advantage). There are roughly 200-250 members at any given time enrolled in this plan with End Stage Renal Disease (“ESRD”).”
2. Would you consider a PMPM proposal, or are you only interested in vendors that are willing to take full risk on this population?
Answer: RFP Section 5.5, Fees, states, “Possible fee basis options:
a. Risk bearing. Describe risk-based arrangements you are willing to take on, or
b. Fixed Fee. Provide a fixed fee to complete the project, or
c. T&M. Provide fees on a time and materials basis with a not-to-exceed price to complete the project, or:
d. Contingency. Provide a contingency fee percentage of the total savings resulting from the data mining services.”
Proposers should provide a fee as listed above; however, Proposers are free to offer fee options in additional to the above.
