About this file
This document is a clarification response from CareOregon, a nonprofit health plan in Oregon, regarding Request for Proposals #2025-06 for End Stage Renal Disease (ESRD) Population Risk-Based Management Services. The RFP seeks a risk-bearing entity to provide comprehensive population health management services for approximately 200-250 ESRD members enrolled in CareOregon Advantage Medicare Advantage plan. Services include designing kidney care programming, ensuring members receive entitled benefits, and implementing population health management for chronic kidney disease through ESRD stages. In response to clarifying questions, CareOregon confirmed that the project focuses on CKD Stage 4 through ESKD populations within the Medicare Advantage (MA) line of business only, excluding DSNP and Medicaid lines. The deadline for clarifying questions was December 22, 2025, with proposal submissions due January 6, 2026, at 2:00 PM Pacific Time. The contract is scheduled to commence in February 2026 for an initial three-year term through December 31, 2028, with options for two additional one-year renewals.
The RFP evaluation will assess proposals across four categories: General Background and Qualifications (0-30 points), Scope of Work (0-35 points), Equity, Diversity, and Inclusion Efforts (0-10 points), and Fees (0-25 points), totaling 100 possible points. Proposals must not exceed 15 pages and be submitted via email to vendorservices@careoregon.org. CareOregon indicated openness to executing mutual non-disclosure agreements to protect proprietary data and sensitive details submitted during the proposal process. The selected contractor must be a risk-bearing entity capable of assuming medical expenses, demonstrate substantial experience with Medicare Advantage and ESRD populations, and maintain comprehensive insurance coverage including Commercial General Liability ($1,000,000 per occurrence/$2,000,000 aggregate), Professional Liability ($1,000,000 per occurrence/$2,000,000 aggregate), and Cyber Liability ($1,000,000 per occurrence). The fee structure accommodates flexible risk-sharing arrangements including full capitation, fixed fees, time and materials pricing, or contingency fees based on savings, with strict HIPAA compliance and adherence to CareOregon's Data Security Requirements required.
