Tie-Breaking Certifications for RFP #11022

This document is a tie-breaking certifications attachment for RFP #11022 issued by the State of Florida's Department of Juvenile Justice for a fifty-bed moderate-risk residential program serving males ages ten to nineteen requiring juvenile sexual offender treatment services. The program will operate in a Department-owned facility located at 9502 Columbus Drive, Tampa, Florida 33619, or in an alternative respondent-owned or leased facility within Florida. The attachment outlines the preference criteria and certifications that respondents may claim to break ties between equal bids or proposals.

The tie-breaking certification establishes five preference categories listed in order of precedence: Drug Free Workplace (per Florida Statutes § 287.087), Certified Minority Business Enterprise (CMBE per § 287.057(11)), Service Disabled Veteran's Business Enterprise (per § 295.187(4)), Florida Business (per § 287.084), and Foreign Manufacturer with a Factory in Florida (per § 287.092). Respondents may check "True" for multiple applicable designations and are required to provide supporting documentation for any certifications marked as true. For CMBE and Service Disabled Veteran's Business Enterprise certifications, respondents must disclose their net worth. The document requires completion of the Drug-Free Workplace Certification (Attachment K) if that box is checked, and all certifications must be signed by an authorized representative with contact information provided.
