About this file
This document is a Foreign Country of Concern Attestation form (PUR 1355) issued under Rule 60A-1.020, F.A.C. applicable to governmental entities in Florida. The form is required for any entity submitting a bid, proposal, or contract that would grant access to an individual's Personal Identifying Information. The attestation requires an officer or representative of the entity to declare under penalties of perjury that their organization is not owned by, organized under the laws of, or has a principal place of business in a Foreign Country of Concern, and that no such government has a controlling interest in the entity.
The form is a standard compliance document designed to ensure that governmental contracts are not awarded to entities with significant foreign government connections that might pose potential security or privacy risks. Entities must complete the form with their printed name, title, signature, and date to affirm their compliance with these requirements. The form references specific Florida Administrative Code rules and is part of a broader governmental procurement and contracting risk mitigation strategy focused on protecting sensitive information and maintaining national security protocols at the state level.
