About this file
The document is a Foreign Country of Concern Attestation form (PUR 1355) for use with governmental entities in Florida, specifically aligned with Rule 60A-1.020, F.A.C. The form requires an officer or representative of an entity submitting a bid, proposal, or contract to attest that their organization is not owned by, organized under the laws of, or principally located in a Foreign Country of Concern, and that no foreign government has a controlling interest in the entity. The form must be completed when an entity is seeking a contract that would grant access to an individual's Personal Identifying Information.
The attestation is made under penalties of perjury, with space provided for the printed name, title, signature, and date of the attesting representative. The form is designed to ensure transparency and protect governmental interests by requiring bidders and contractors to disclose and certify their organizational independence from specified foreign governmental influences. It is part of a broader governmental procurement compliance process that aims to safeguard sensitive information and maintain the integrity of state and local contract opportunities.
