1. Can your company provide the product or service? Please provide an explanation.
2. What NAICs and FSC/PSC do you recommend for this?
3. Is your company a small as defined by the Small Business Administration and under what NAICs?
4. As the requirement is described, what is the estimated costs for such work?
5. If your company were selected, how many calendar days would your company need after contract award date or notice to proceed, before you could be completely set-up and begin providing services?
6. Please explain your pickup and delivery processes/requirements (e.g. delivery time).
7. What standards does your company use to validate quality assurance?
8. Does your company have a satisfactory performance record for the type of work required? Please provide examples of relevant and recent (within three years) past performance.
9. Provide any additional feedback that you feel is relevant?
10. Are you able to provide a quote fora base plus two (2) to four (4) option years?
