Request for Information (RFI): USU Hazardous Waste Collection Services
THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes and does not constitute a solicitation or issuance thereof.

Proposed North American Industry Classification System (NAICS): 562112 – Hazardous Waste Collection
Proposed Product and Service Code (PSC): S222 Housekeeping – Waste Treatment/Storage
The USU is issuing this RFI to obtain information regarding the availability and capability of qualified small businesses to support the requirements delineated herein. Please refer to the “Scope” and “Information Requested” sections of this RFI for specific information that should be detailed in any capability statement submission.
Background
The USU is a Department of Defense (DoD) agency and medical research/teaching facility. The USU is located on the grounds of the Naval Support Activity Bethesda (NSAB) in Bethesda, Maryland. The USU mission is to educate, train, and comprehensively prepare uniformed services health professionals, scientists, and leaders to support the Military and Public Health Systems, the National Security and National Defense Strategies of the United States, and the readiness of our Uniformed Services.
The USU does not own or operate an Environmental Protection Agency (EPA)-permitted treatment, storage or disposal facility. Instead, the USU is a large-quantity generator of hazardous/acutely-hazardous wastes with its own EPA Generator ID Number accumulating hazardous/acutely hazardous wastes on site in compliance with Code of Maryland Regulation (COMAR) 26.13.03.05E and local, state, or federal statutes and regulations.
As a medical school that includes several multi-disciplinary research laboratories, the hazardous/acutely hazardous and regulated wastes generally consist of excess/surplus/spent laboratory chemicals.
Scope
The USU is seeking information on hazardous waste collection services to support its facility, to include the Armed Forces Radiobiology Research Institute (AFRRI) Complex. Support is anticipated to include the inspection of materials packaged and prepared by the USU to confirm compliance with Code of Maryland Regulation (COMAR) 26.13.03.05E and local, state, or federal statutes and regulations.
Trained, qualified personnel are necessary for the preparation, labeling, placard, manifesting, transporting and disposal of waste materials.
The Government anticipates awarding a Blanket Purchase Agreement (BPA) with an ordering period of five years beginning on or about January 31, 2022.
Any prime contractor, subcontractor, or other personnel must be without exclusions, debarment or other performance issues as listed in the System for Award Management (https://www.sam.gov), the Contractor Performance Reporting System (https://www.cpars.gov), and/or the Federal Awardee Performance and Integrity Information System (FAPIIS).
Information Requested
To respond to this RFI, please provide the following information within the capability statement.
1. Organization Information.
a. Name and address of organization.
b. Name, title, phone number, and email address of business and technical points of contact.
c. Data Universal Numbering System (DUNS) and/or Unique Entity Identifier
(UEI).
d. Commercial and Government Entity (CAGE) code.
e. Indicate your size and socio-economic status, including (but not limited to) any of the following designations: Alaska Native Corporation, Indian Tribe, Native Hawaiian Organization, etc.
f. Indicate whether you are a small business under the NAICS code 562112 – Hazardous Waste Collection. Note: To be considered for a small business set-aside, at least 50% of the cost of contract performance incurred for personnel shall be expended for employees of the responder. (FAR 52.219-14 Limitation of Subcontracting)
g. If you are an 8(a) small disadvantaged small business under the NAICS code 562112 – Hazardous Waste Collection, provide entry date into the SBA’s 8(a) program.
h. Type a DoD Facility Clearance, if applicable.
2. Review the proposed North American Industry Classification System (NAICS) and Product Service Code (PSC). If you wish to propose an alternative NAICS or PSC, please include the proposed code(s) and rationale/justification for the proposed alternative(s).
3. Brief summary on experience providing the same or similar services as outlined herein.
4. Identify the organization(s) you have provided or currently provide the same or similar services to. Information provided should be within the last 3 years. For each organization identified, provide the following:
a. Contract name
b. Contract number https://www.sam.gov/ https://www.cpars.gov/
c. Total award amount
d. Period of performance (including any options)
e. Agency (or business) contract awarded by
f. Agency (or business) point of contact information to include name, title, phone number, and email address
g. Indicate whether your organization was a prime or subcontractor
5. If you hold a GWAC contract applicable to this type of work.
6. Provide any comments, feedback, requests for clarification, or suggestions regarding the draft Statement of Work and Price Table.
Response and Process
Interested parties are requested to provide a capabilities statement.
RFI Responses shall be submitted by 4:00PM (Eastern) on Friday, December 17, 2021.
Responses should be limited to no more than 10 pages, and must be submitted via email to the following individuals.

DO NOT submit pricing information in response to this RFI. General marketing information or marketing flyers will be not be reviewed.
DO NOT include proprietary, classified, confidential, or sensitive information in any part of the RFI response. DO NOT submit cost related or pricing information in response to this RFI.
Any requests for clarifications or questions will not be responded to by the submission date of this RFI. Any clarifications/questions submitted will be addressed in a final Statement of Work as part of any resultant solicitation.
