About this file

This document is Addendum 1 for Request for Bid (RFB) 931600-01, issued by the Iowa Department of Administrative Services (DAS) for the CHMHI New Parking Area project at Cherokee Mental Health Institute. The addendum addresses contractor questions regarding project specifications, including confirmation that the contractor is responsible for staking, there are no liquidated damages, and the project will be bid as a single package. The document provides paving quantities for reference, including 1,020 square yards of 6" PCC paving, 60 square yards of 5" general PCC paving, and 50-70 square yards of 5" PCC sidewalk (base and alternate bids).

Story Construction will serve as the construction manager agent, and bidders are instructed to submit proposals through the IMPACS Electronic Procurement System. The addendum clarifies that the winning bidder will be the prime contractor managed by Story Construction. A pre-bid meeting is scheduled for Wednesday, October 4th at 11:00 am, after which an addendum will be issued listing all attending contractors. The quantities provided are for reference only, and contractors are expected to verify all dimensions and quantities, with the understanding that no additional compensation will be provided for discrepancies or additional materials required to meet the project intent.
