About this file

This is an Application and Applicant's Information and Certification Sheet (Attachment A) for the Oregon Department of Human Services (ODHS) Child Welfare - Adoption Vendor Attorney Services solicitation (S-10000-00011992). The document outlines the application process for attorneys seeking to be listed on the Adoptions Services Vendor Attorney Registry, which is provided to Adoptive Providers to assist them in selecting vendor attorneys across Oregon's 36 counties. Applicants must be active members of the Oregon State Bar in good standing, maintain required insurance coverage, and have a minimum of three years' experience providing legal services for adoptions.

The application requires attorneys to certify compliance with various state requirements including non-discrimination policies, wage equity laws, tax compliance, and conflict of interest disclosures. They must also certify they are not included on federal exclusion lists or the Specially Designated Nationals and Blocked Persons list. The application constitutes a firm offer valid for 180 days after closing. Notably, contracts with anticipated values over $150,000 require additional certifications regarding sexual harassment and discrimination prevention policies as described in ORS 279A.112.
