Claremore Indian Hospital Indian Health Service US PHS Indian Hospital 101 South Moore Avenue Claremore, Oklahoma 74017

SCOPE OF WORK

Emergency OR Flooring Repair September 27, 2022 I. Objective:

The objective of this emergency service is to repair the floor and replace flooring material in multiple locations in the OR department at the US PHS Indian Hospital located in Claremore, Oklahoma. The hospital is a 110,000+ ft2 hospital building that consists of outpatient services such as OB/GYN, Behavioral Health, Pediatrics, Medical/Urgent Clinics, Contract Health, Dental, WIC, Pharmacy, Diabetes Education, and Public Health, as well as in-patient services that includes ER, OB, in-patient, OR, and ICU.

II. Background:

The OR floor was originally constructed with concrete subbase with a black terrazzo floor finish. Sometime in the 1990’s the terrazzo floor finish in OR suites A, B, and OR interior hallway was covered by commercial-grade homogeneous sheet vinyl flooring with heat-welded seams. Multiple repairs have been performed since then to replace broken concrete flooring, reseal heat-welded seams, and ripped sheet vinyl flooring in both OR suites. The same issues are still occurring and currently OR suites A & B floor in the middle of the room where the surgery table is placed has ripped heated-welded seams and tears in the floor that have allowed liquids to penetrate and disintegrate the subfloor underneath (self-leveling product?) and create uneven floor surfaces. Other flooring issues are present with multiple rips and tears in both of these rooms and in the OR hallway, as well as sections of flooring in the OR suites that are showing ruts from gurney traffic and additional subbase issues. The existing sheet vinyl flooring company is no longer in business and the product line has been dropped from the business that acquired the holdings. This requires all of the existing sheet vinyl flooring to be replaced ( III. Scope: Reference “Attachment A Existing OR Floor Conditions and Layout” and “Attachment B. OR Floor Plan” for all work A. General:

2. Contractor shall provide all labor, materials, supplies, tools and equipment, transportation, supervision and all other necessary items for repairing/replacing the flooring in the OR department.
