STATEMENT OF WORK
FOR
Servicing of Restroom Trailers
Hurlburt Field 1 SOMXS
4 February 2020
STATEMENT OF WORK
RESTROOM TRAILER UNITS & SERVICES
HURLBURT FIELD, FL
1. SCOPE OF WORK: The contractor shall provide all personnel, equipment, tools, materials, supervision, licenses, quality control, and other items and services necessary to complete this contract. Contractor shall comply with all federal, military, environmental, and state and local laws and regulations. The contractor shall provide services to maintain the usability of the units.
The equipment must be able to sustain 24/7 accessibility and usage. Each Restroom Trailer will be serviced at a minimum once per month and as then serviced, pumped and refilled with water as needed to be fully functional at the discretion of the government point of contacts or the contracting officer. The unit has a 125 gallon fresh water tank and a 370 gallon waste water tank. There is two half bath stalls with toilet and sink.
2. HOURS OF OPERATION: All Restroom Trailers
3. LOCATION: All work on this contract shall be performed at the Hurlburt Engine Test Cell 665 Redhorse Road Building 90581 and 1 SOMXS AGE 740 Cruz Ave 90287 Hurlburt Field Fl, 32544.
4. DESCRIPTION OF WORK: All Restroom Trailer units are to be serviced, pumped and refilled with water by the contractor during the performance period to maintain equipment to be fully functional.
The following outline of the principal features of work does not in any way limit the responsibility of the contractor to perform all work and furnish all materials required for a complete job within the intent and scope of the contract documents.
4.1 Contractor shall pump or evacuate the effluent from the Restroom Trailer receptacles into the truck holding tank.
5. SITE CLEAN UP: The contractor will dispose all waste off-site.
6. EQUIPMENT: The contractor shall provide all equipment and supplies required to complete the work, except for items specifically identified as being government furnished. Any government items provided to the contractor will be identified and if they are not deemed as being expendable they will be returned to the government in good working or usable condition as issued.
Expendable items provided to the contractor by the government due to poor planning of stock will be reimbursed to the government by means of a deduction in the final billing using local market value.
7. VEHICLES: The contractor shall provide all vehicles necessary to complete this contract. The contractor shall be able to transport all equipment, materials, supplies, and personnel to and from the job site to perform the required services.
8. WORKING CONDITIONS: All work shall be performed to minimize interference to existing user(s). The contractor shall comply with all federal, state, and local laws pertaining to safety and fire codes.
9. CONTRACTOR EMPLOYEES: The contractor’s foreman or representative(s) shall present a neat appearance and shall have the capability of speaking and understanding the English language. The contractor shall ensure all employees possess proper credentials allowing them to work in the United States. Additionally, the contractor will ensure illegal aliens are not employed and/or transported onto the installation. The Contractor shall identify an employee who has the authority to act on all contractual matters.
The employee shall be present and available during normal duty hours. The Contractor shall not employ persons for work on this contract who are identified to the Contractor by the CO as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population.
10.0 SPECIAL REQUIREMENTS: The contractor will coordinate service(s) with the government point of contacts or the contracting officer within 1 days of the request of additional services. Additionally, the contractor will also provide the contact information of the stand-by maintenance teams to the POCs at this time.
11.0 BASE SECURITY REQUIREMENTS. The contractor’s employees, performing portable toilet and/or Restroom Trailer pumping and cleaning services within Hurlburt Field, shall obtain a security pass. The contractor must provide upon award of this contract a list of employees, to include social security number, first, middle, and last name to the contracting office. This information will be provided no later than 5 days prior to the delivery date. The contracting office will then submit this information to the Security Forces and Pass/Registration for processing using proper form(s) and/or letter(s). Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles to be registered that are coming on-base. The contractor shall make sure all employees have the required training, certifications and comply with all federal, state, and local environmental requirements and/or laws.
12.0 SAFETY. The Contractor shall conform to all safety regulations to include but not limited to: Occupational Safety and Health Administration (OSHA).
