About this file
The Responsible Contractor Questionnaire Form A is a document from the City of Royal Oak related to the Farmers Market Restroom Facility project, with electronic proposals being accepted through the procurement portal until 5:00 pm on Thursday, December 12, 2024. The City reserves the right to reject any proposals and select the most favorable option, with award results to be made available through the procurement portal after proposal evaluations are completed.
The questionnaire requires detailed company information including contact details, ownership, past business names, and potential control by other entities. Contractors and subcontractors must submit additional documentation such as financial statements, state and local licenses, insurance proof, litigation history, and statements affirming compliance with wage and benefit laws. The form is designed to comprehensively assess a bidder's financial capacity, legal standing, and ability to perform the contract, with specific requirements for transparency about the company's background and operational capabilities.
