About this file
This is a collection of required solicitation forms and certifications for contractors doing business with the California Department of State Hospitals - Patton. The forms package includes the Darfur Contracting Act Certification, Payee Data Record (STD 204), Postconsumer-Content Certification, Contractor Certification Clauses (CCC 04/2017), Disabled Veteran Business Enterprise Declarations, Voluntary Statistical Data Sheet, and Seller's Permit/Certification of Registration requirements.
The forms require various certifications and disclosures from contractors, including verification that they are not a scrutinized company under the Darfur Contracting Act, documentation of recycled content in products, compliance with drug-free workplace requirements, confirmation of worker's compensation coverage, and proof of California seller's permit status. Key submission requirements include providing a Federal ID Number, completing certifications under penalty of perjury, and submitting voluntary demographic data about business ownership. The forms package appears designed for general contracting use rather than being specific to a single procurement opportunity.
