About this file

This file contains required bidding forms for a Virginia state contract opportunity for a freight elevator addition at Virginia Industries for the Blind in Charlottesville. The primary documents included are a Small Business Subcontracting Plan, a Capital Outlay Vendor Qualification Certification Form, and a State Corporation Commission registration form. Bidders are required to submit a subcontractor plan demonstrating their utilization of Virginia Department of Small Business and Supplier Diversity (DSBSD) certified small businesses, with the Commonwealth's goal being that over 50% of purchases be made from small businesses. The subcontractor plan must identify the portions of the contract to be subcontracted to DSBSD-certified small businesses in relation to the bidder's total price for the initial contract period. Bidders certified as micro or small businesses by DSBSD need only complete Section A, while non-certified bidders must complete Section B, which allows for identification of up to five subcontractors with their certification numbers, contact information, and work descriptions.

All bidders must complete the Capital Outlay Vendor Qualification Certification Form, providing business information including current employment, existing projects, vendor status with Commonwealth agencies, and three customer references for similar work. Bidders must provide State Corporation Commission registration information or demonstrate exemption status as an out-of-state entity. Failure to submit the required subcontractor plan and forms, or to return the Capital Outlay Vendor Qualification Certification Form within two business days of request, may result in the bid being deemed non-responsive. The proposed subcontractor participation identified in the bid will be incorporated into the subsequent contract and become a contractual requirement, with failure to achieve proposed participation levels potentially resulting in breach of contract.
