About this file

This document is a Required Attachments Checklist and supporting forms for a Request for Proposal (RFP) entitled SFBC-P-25-14 for Strategic Planning Services issued by the San Francisco Bay Conservation and Development Commission (BCDC). The checklist outlines nine required attachments that all proposers must submit to be considered responsive, including the Proposal/Proposer Certification Sheet, Proposer References (three references for similar services performed within the last five years), Payee Data Record (Std 204), Contractor Certification Clauses, Bidder Declaration, Commercially Useful Function Certification (if applicable), Disabled Veteran Business Enterprise Declaration (if applicable), and Darfur Contracting Act Certification (if applicable). The document specifies that proposals must be submitted in triplicate with original signatures in a sealed envelope in accordance with RFP instructions, and that all required attachments must be included for the proposal to be deemed responsive and not subject to rejection.

The document establishes mandatory compliance requirements for contractors, including nondiscrimination program compliance, drug-free workplace certification, National Labor Relations Board compliance, and applicable state and federal labor code requirements. Proposers must provide complete company information, organization type, federal and state identification numbers, and relevant business licenses or certifications. The documentation requires certification of California residency status for tax and payment purposes through the Std 204 form, verification of compliance with conflict of interest provisions regarding current and former state employees, and declarations regarding corporate qualification to do business in California. For small businesses, microbusinesses, and Disabled Veteran Business Enterprises, proposers must demonstrate they are performing a commercially useful function as defined by state law, and DVBE proposers must complete specific declarations regarding ownership percentages and equipment rental provisions. All certifications are made under penalty of perjury, and false certifications may result in contract suspension, termination, or ineligibility for future state agreements.
