About this file
This document is a Request Form to the Idaho Division of Purchasing (DOP) from the Idaho Division of Veterans Services. The agency is requesting an exemption from competitive procurement for the one-time purchase of Duopa medication for a resident of the Idaho State Veterans Home - Pocatello who suffers from Parkinson's Disease. The medication is only available from three vendors - Pharmacy Solutions, CVS, and Accredo - and is required in liquid form for the resident's treatment. The agency cites the impracticality of competitive bidding, as the availability of the medication from a single vendor may pose a detrimental risk to the resident's health and well-being.
The anticipated total cost for the purchase is $83,000. The agency is requesting the ability to purchase the medication from any of the three identified vendors to ensure availability. DOP is offering suppliers the opportunity to review the requested exemption and provide relevant information, although the final decision remains at the discretion of the DOP Administrator.

Describe the circumstances
The Idaho State Veterans Home - Pocatello has a resident who suffers from Parkinson's Disease. Duopa is a medication that is delivered to him in the form of a liquid which is contained in a cartridge that is then inserted into a device called a Duopa Pump. A tube that has been surgically inserted into the Resident's small intestine or stomach is connected to the pump thereby providing the medication in a more consistence manner and provides improvement of the Resident's symptoms throughout each day. The Resident is unable to take the tablet form of this medication offered by McKesson because of the severity of his Parkinson's disease and the need for around the clock medication, which has been prescribed by a neurologist. Research conducted by the Pharmacist in Pocatello has found that only 3 vendors (Pharmacy Solutions, CVS, and Accredo) provide this medication, in a liquid state.
Why would the competition be impractical, unreasonable, or disadvantageous under the circumstances?
Given that only 3 vendors' carry this medication in liquid form, it would be unreasonable to assume that one vendor will always have this medication readily available and in stock.

Describe the extent to which competition is possible
Competition would be limited to the 3 vendors mentioned in this request. Providing a contract to only one vendor may put the Home at risk of not being able to purchase this medication should one vendor run out of stock and pose detrimental risk to the Resident's health and wellbeing. Therefore, the Idaho State Veterans Home - Pocatello is requesting the ability to purchase this medication from any of the 3 vendors identified in this request.
The information provided above is true and accurate to the best of my knowledge, and supports the need for a direct procurement of the described goods or services, in the best interest of the State:
