About this file
This is a Recycled Material Content Questionnaire and Affidavit form used by Collin County, Texas in connection with a road materials procurement opportunity for cement treated base and cement stabilized backfill. The document is designed to ensure compliance with the county's preference for recycled products in accordance with the Comprehensive Procurement Guideline (CPG) standards established by the Environmental Protection Agency. Suppliers responding to this procurement must complete the questionnaire for each line item, identifying the relevant section of the Recovered Materials Advisory Notice (RMAN), the percentage of recovered material contained in the product, the percentage of post-consumer material, and whether the product meets the specific recommendations for the designated item. Bidders must provide supporting documentation to verify claimed recycled content.
The affidavit portion requires the supplier's authorized representative to swear or affirm under penalty of perjury that all information provided is true and accurate, and that the supplier commits to maintaining the stated recycled material content throughout the contract term. Suppliers also agree to permit Collin County to audit their files to confirm ongoing compliance and must notify the county within 30 days of any changes to the recyclable content of materials cited in the affidavit. This document serves as a binding commitment mechanism to ensure that products supplied meet the county's environmental procurement standards and that suppliers maintain transparency and accountability throughout the contract period.
