About this file

This is a Recycled Material Content Questionnaire and Affidavit form required by Collin County, Texas for suppliers bidding on the Safety Wear procurement (IFB 2026-318). The form mandates that bidders document the recycled material content of products being offered, specifically requiring suppliers to reference the EPA's Consolidated Recovered Materials Advisory Notice (RMAN) for the Comprehensive Procurement Guideline (CPG). Suppliers must complete a detailed table for each product line item, identifying the applicable RMAN section, the percentage of recovered material content, the percentage of post-consumer material content, and whether the product meets the specific procurement recommendations for designated items. Bids for the Safety Wear contract must be submitted by Thursday, August 6, 2026 at 2:00 PM CST, with award going to the lowest responsive and responsible bidder meeting all required specifications.

By signing the affidavit, suppliers certify that all information provided regarding recycled content is accurate and agree to maintain compliance with the stated recycled material percentages throughout the contract term. Suppliers must also consent to audits by Collin County to verify compliance and commit to notifying the county within 30 days of any changes to the recyclable content cited in the affidavit. This requirement reflects Collin County's preference for environmentally sustainable products and establishes accountability mechanisms to ensure suppliers honor their material composition commitments. Contact for inquiries regarding the Safety Wear procurement is Jingna Lyu in the Collin County Purchasing Office.
