Questions and Answers 1.Competency Validation This is listed as including fingertip sampling, media-fill, and didactic review with a 10-person minimum.

Can you please confirm that this is equivalent to full employee qualification, including: Yes, this is correct. I believe this is a line item for ongoing competency and is really the same as the line item for new employee competency validation (full employee qualification).

• Initial fingertip sampling

• Media-fill

• Post-media-fill fingertip sampling

• Post-media-fill surface sampling

2. 3× Fingertip Sampling for New Employees Can you confirm this refers to the initial three-round fingertip qualification process for new personnel, including all plates, shipping, incubation, and reporting? Yes, this is correct. I am thinking this is broken out as a separate line item in case an employee fails and has to repeat the garbing competency (must get 3 in a row without growth)

3. New Employee Competency Validation This is listed as including fingertip sampling, media-fill, post-media-fill surface sampling, and didactic review with a 10-person minimum.

Can you please confirm this includes all of the following: Yes, this is correct.

• 3× fingertip sampling

• Media-fill

• Post-media-fill fingertip sampling

• Post-media-fill surface sampling

• Didactic review/training

4. Competency Training Done In-House Since this appears to include very similar components to the first competency validation listing, could you clarify how this service differs? This is the cost for just the in-person required initial Training and Competency in Maintaining the Quality of the Sterile Compounding Environment required for new employees (but not required for ongoing training). It’s confusing b/c it states that media fill, etc. are provided but that is actually charged in other line items.

5. Pricing Minimum For the items that specify a 10-person minimum, should we provide pricing:

• Per individual (e.g., $200 per person) Yes, please provide a per-person cost.

or As a group rate for 10 individuals (e.g., $2,000 total)? No

6. The spreadsheet is currently set with an automatic 5% year-over-year increase. Are we permitted to adjust this percentage? I don’t know about this one but I assume they can adjust the % because they are providing the quote to us. Can they adjust the cost of the service upward each year of the contract?

Several items show a quantity of zero. Would you like us to still provide pricing for these items in case they are added in the future? Yes

7. Regarding the hood pricing: there is a single line item for retesting each hood, but costs vary by hood type. For example, a laminar flow clean bench and a biosafety cabinet have different recertification prices. How would you like us to differentiate pricing among different hood types? The worksheet that I am aware of does break out the types of hoods as follows and yes, they may have different certification values. So unclear about what this is in reference to.

8. Is there a previous contract for this work? If so, could you please provide the contract number? Yes, 36C25521D0019.
