About this file
This document is a Quality Assurance Surveillance Plan (QASP) for a Warehouse Personnel Contract supporting the U.S. Coast Guard (USCG) Auxiliary National Supply Center in Granite City, Illinois. The plan outlines surveillance methods for monitoring warehouse inventory, logistics, and customer services, with key government personnel including Kenechukwu Anakor (Contracting Officer) and Tom Dardis (Contracting Officer's Representative). Performance standards include shipping all orders within five business days, providing monthly inventory and mailing reports, and promptly reporting damages or shipment inconsistencies.
The QASP establishes a performance rating system with three levels (Exceptional, Satisfactory, Unsatisfactory) and details consequences for unacceptable performance. The Contracting Officer's Representative will conduct quarterly performance assessment meetings and can issue Contract Discrepancy Reports for repeated failures to meet performance standards. Incentives and disincentives are tied to the contractor's ability to meet specified performance requirements, with potential impacts on future contract options and past performance evaluations.
