About this file
This is a Quality Assurance Surveillance Plan (QASP) for the VA Pacific Island Health Care System's Health Care for Homeless Veterans (HCHV) Emergency Housing program on Maui, Hawaii. The QASP establishes the systematic framework for evaluating contractor performance against 15 specific performance standards, with all scores required to meet or exceed 95%. The Government, through the Contracting Officer (CO) and Contracting Officer's Representative (COR), will monitor compliance through direct observation, periodic inspections, random sampling, participant satisfaction reviews, and analysis of contractor progress reports.
Key performance requirements include maintaining 80% bed occupancy, achieving same-day admissions 100% of the time, completing comprehensive assessments and Housing Support Plans within seven days of admission, submitting monthly housing support summaries documenting at least three meetings per veteran monthly, achieving 55% discharge to permanent housing and limiting negative discharges to 20% or less, limiting emergency housing stays to 90 days unless authorized, maintaining a case manager-to-veteran ratio not exceeding 1:15, ensuring 24/7 staffed site operations, providing structured group activities at least twice monthly, maintaining facility standards, and providing three nutritious meals daily plus one snack. The COR will conduct quarterly performance assessment meetings with the contractor and document all findings through Contract Performance Reports. Unacceptable performance (below 90% rating) triggers a Contract Discrepancy Report requiring the contractor to submit a corrective action plan within a specified timeframe. This document also includes templates for performance reporting and discrepancy documentation.
