About this file
This document is a Quality Assurance Surveillance Plan (QASP) for a contract for solid waste removal and recycling services for the VA Harbor Healthcare System and affiliated facilities. The QASP outlines the performance standards, acceptable quality levels, and methods of surveillance that the government will use to monitor the contractor's performance. Key performance indicators include timely replacement of damaged containers, monthly reporting on waste and recycling quantities, and timely and accurate invoicing. The government will use periodic inspections, customer complaints, and contractor ratings of excellent, satisfactory, or unsatisfactory to assess performance. Unacceptable performance will be documented through Contract Discrepancy Reports, and the contractor will be required to submit a corrective action plan. The QASP is a "living document" that may be revised by the government in coordination with the contractor.
The related federal contract opportunity is for the same solid waste removal and recycling services, with a solicitation number of 36C24224Q0632. The contracting agency is the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2.
