About this file

This document is a prequalification instruction form (FNG 5085) issued by the State of Florida, Department of Military Affairs, for contractors bidding on construction projects classified as Level Four and Five contracts. The form establishes requirements that qualified, licensed contractors must submit with their bids prior to contract award. The document specifies qualification submission procedures and required documentation to expedite contract processing for the CBJTC 3600 Area Paving project located at Camp Blanding Joint Training Center in Starke, Florida. The project, with an estimated construction cost of $467,000, involves repaving identified streets, replacing culverts where paving crosses them, and removing grass from roadway edges and driveways. The solicitation date is July 22, 2026, and contractors are required to obtain and pay for local building permits, with the exception of Fire Marshal permits which remain the state's responsibility.

Contractors submitting bids exceeding $500,000 (Level Five) must provide a Florida State Contractor License, corporate charter number, proof of an active office within 300 road miles of the project, project manager and superintendent resumes meeting regulatory requirements, documentation of at least two successfully completed projects of similar scope and complexity within the previous five years, vendor registration in MyFloridaMarketplace, and a bonding company letter of intent confirming ability to bond the project. All contractors must disclose project history for the 36 months preceding solicitation, explain any variations between original and final contract prices or completion dates, and identify any liquidated damages, penalties, liens, defaults, or cancellations. Projects submitted for qualification must be completed work, not subcontractor work, and contractors must specify whether projects involved new construction or renovation.
