About this file
This is a Contractor's Information Sheet for a federal subsistence supply contract opportunity with the Federal Bureau of Prisons at FCI Leavenworth. The solicitation number is 15B40826Q00000020 and covers subsistence items for Fiscal Year 27 Quarter 1.
The document is a template requiring contractors to submit company information including name, Unique Entity ID, CAGE Code, point of contact details, email address, and phone number. Contractors must acknowledge compliance with the National Menu Specifications Quote Sheet, which mandates that all delivered items comply with Federal, State, and local regulations governing preparation, packaging, labeling, storage, distribution, and sale of food supplies. Items must meet all provisions of the Federal Food, Drug, and Cosmetic (FD&C) Act and the Fair Packaging and Labeling Act, including allergen labeling requirements for the nine major allergens (wheat, fish, milk, soy, tree nuts, eggs, peanuts, shellfish, and sesame). Items may originate from foreign countries if not specified as U.S. or Canadian products, and no item may be produced, processed, or packaged more than six months prior to delivery. Vendors are solely responsible for ensuring quoted items meet all specifications; providing labels or images does not exempt vendors from specification compliance, and award of an item does not constitute government acceptance of any out-of-specification products. Signature and date certification is required on submission.

PER NATIONAL MENU SPECIFICATIONS QUOTE SHEET:
The delivered item(s) must comply with all applicable Federal, State, and local mandatory requirements and regulations relating to the preparation, packaging, labeling, storage, distribution, and sale of the supplies in the commercial marketplace. Delivered item(s) must comply with all applicable provisions of the Federal Food, Drug, and Cosmetic (FD&C) Act, the Fair Packaging and Labeling Act, and regulations promulgated thereunder. When a known allergen is included in the item(s), the item(s) must comply with the allergen labeling requirements of the FD&C Act. Major allergens identified in the FD&C Act include: wheat, fish, milk, soy, tree nuts, eggs, peanuts, shellfish, and sesame or those in effect on the date of the solicitation, contract, or purchase order. Failure to comply with the National Menu Specifications will result in your quoted item not being considered for award. No specification substitutions will be accepted. If a specification does NOT state it must be a product of the United States or Canada, it is acceptable for it to be a "product of" a Foreign Country and distributed in the United States or Puerto Rico. - If not specified elsewhere, no item may be produced / processed / packaged more than six (6) months prior to date of delivery. The government requires all quoted items to meet all specifications for the respective item; it is the vendor's responsibility to ensure that the quoted (supplied - if awarded) item meets all specifications. The attachment of images, labels, or other documents to supplied quotes do not excuse / exempt vendors from their responsibility to supply items within government specifications, nor does the award of any item that may have had an attachment indicating the item(s) failure to meet specifications constitute the governments acknowledgment or intent to accept any out of specification item.
