About this file
This is a Contractor's Information Sheet for a Federal supply solicitation (Contract Number 15B40826Q00000021) for halal food items at FCI Leavenworth for Fiscal Year 27 Quarter 1. The document serves as a template for contractors to submit company identification information, including company name, Unique Entity ID, CAGE Code, point of contact details, email address, and phone number.
The sheet establishes mandatory compliance requirements for all quoted items, which must meet Federal, State, and local regulations governing food preparation, packaging, labeling, storage, distribution, and sale. All items must comply with the Federal Food, Drug, and Cosmetic (FD&C) Act and the Fair Packaging and Labeling Act, with particular attention to allergen labeling for the nine major allergens (wheat, fish, milk, soy, tree nuts, eggs, peanuts, shellfish, and sesame). The government requires that no item may be produced, processed, or packaged more than six months prior to delivery, and vendors are responsible for ensuring all quoted items meet specifications regardless of any attached documentation or images. No specification substitutions are permitted, and items may be foreign-produced and distributed in the United States or Puerto Rico unless otherwise specified. The document includes a signature line for contractor certification of compliance with these requirements.

PER NATIONAL MENU SPECIFICATIONS QUOTE SHEET:
The delivered item(s) must comply with all applicable Federal, State, and local mandatory requirements and regulations relating to the preparation, packaging, labeling, storage, distribution, and sale of the supplies in the commercial marketplace. Delivered item(s) must comply with all applicable provisions of the Federal Food, Drug, and Cosmetic (FD&C) Act, the Fair Packaging and Labeling Act, and regulations promulgated thereunder. When a known allergen is included in the item(s), the item(s) must comply with the allergen labeling requirements of the FD&C Act. Major allergens identified in the FD&C Act include: wheat, fish, milk, soy, tree nuts, eggs, peanuts, shellfish, and sesame or those in effect on the date of the solicitation, contract, or purchase order. Failure to comply with the National Menu Specifications will result in your quoted item not being considered for award. No specification substitutions will be accepted. If a specification does NOT state it must be a product of the United States or Canada, it is acceptable for it to be a "product of" a Foreign Country and distributed in the United States or Puerto Rico. - If not specified elsewhere, no item may be produced / processed / packaged more than six (6) months prior to date of delivery. The government requires all quoted items to meet all specifications for the respective item; it is the vendor's responsibility to ensure that the quoted (supplied - if awarded) item meets all specifications. The attachment of images, labels, or other documents to supplied quotes do not excuse / exempt vendors from their responsibility to supply items within government specifications, nor does the award of any item that may have had an attachment indicating the item(s) failure to meet specifications constitute the governments acknowledgment or intent to accept any out of specification item.
