QUALITY ASSURANCE SURVEILLANCE PLAN

Pest Management (Birds, Bats, and Bat Bugs)

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

• Who will conduct the monitoring

• What will be monitored

• How monitoring will take place and the results documented

This QASP does not detail how the contractor accomplishes the work. The contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations might occur.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Updates shall ensure that the QASP remains a valid, useful, and enforceable document. The contractor and Government officials implementing surveillance activities will receive copies and revisions of the QASP.

The following FAR clause applies to this contract:

52.246-4 Inspection of Services – Fixed-Price

2. GOVERNMENT ROLES AND RESPONSIBILITIES

The following personnel shall oversee and coordinate surveillance activities.

a. Contracting Officer (KO) - The KO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and safeguard the interests of the United States in the contractual relationship. The KO shall also make certain that the contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

COR: Emily Kohl

Assigned KO: Mary Fronck Organization or Agency: USACE, San Francisco Telephone: (415) 503-6554 Email: Mary.Fronck@usace.army.mil mailto:Mary.Fronck@usace.army.mil

b. Contracting Officer’s Representative (COR) - The COR is responsible for technical administration of the contract and shall assure proper Government surveillance of the contractor’s performance. The COR shall keep a quality assurance file. At the conclusion of the contract or when requested by the KO, the COR shall provide documentation to the KO. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The contractor shall refer any changes they deem may affect contract price, terms, or conditions to the KO for action.

3.CONTRACTOR REPRESENTATIVES

The following serve as the COR’s Field Representatives, Inspectors, and additional inspectors for this contract.

a. COR’s Field Representative (technical/alternate) – Brian Emmons Telephone: 707-431-4552

b. Additional Contractor Representative Inspectors – Any other permanent or seasonal/temporary staff assigned and properly trained to conduct inspections by the COR.

4. PERFORMANCE STANDARDS

Performance standards define desired services. Contractor is responsible for establishing and maintaining adequate quality control to insure satisfactory performance of contract work in accordance with specifications.

This feature will be assured by a comprehensive Quality Control Plan developed by the Contractor and approved by the COR or designated representative prior to initiating work.

5. METHODS OF QUALITY ASSURANCE SURVEILLANCE

The COR shall use the surveillance methods listed below in the administration of this QASP.

The COR shall be responsible for monitoring the contractor’s performance in meeting specific performance standards. The COR shall always contact the contractor's task manager or on-site representative when a defect is identified and inform the manager of the specifics of the problem.

Quality Control Inspection Reports: The Contractor shall maintain a current record of all Quality Control Inspection Reports. These reports shall be furnished to the COR/COTR each month or more often, if requested in writing. The records of inspections shall cover all work items scheduled and shall be signed and dated by the Contractor’s Quality Control Inspector. The frequency of these inspections shall be of an adequate number to ensure all contract specifications are met. The COR/COTR reserves the right to require daily inspection reports if deemed necessary.

6. INSPECTION DEFICIENCIES: The COR/COTR reserves the right to request the Contractor to return to the job site and address any items which fail to meet the technical cleaning specifications. Any deficiencies must be corrected by the contractor not less than two hours after being notified. Payment will not be made for work which was not performed or does not meet the specifications in the scope of work. After the initial two-hour notification, the Corps of Engineers reserves the rights to have a third party perform any scheduled work which

Assigned COR: Emily Kohl Title: Operations Park Ranger Telephone: 707-431-4517 Email: Emily.T.Kohl@usace.army.mil mailto:Emily.T.Kohl@usace.army.mil was not done or has not been done satisfactorily. In such a case, the government will charge these third-party costs to the contractor.

7. MEETINGS

Performance Evaluation Meetings: The contractor shall meet with the COR/COTR, as often determined necessary by the COR/COTR to discuss performance.

Contract Deficiency Report (CDR): A meeting shall be held no later than one normal workday after a CDR is issued. Written minutes of these meetings shall be prepared and signed by the COR/COTR and the Contractor. Mutual effort will be made to resolve any and all problems identified. Should the Contractor not concur with any decision, etc., the Contractor shall so state, in writing and request a final decision by the Contracting Officer.

8. RATINGS

Metrics and methods are designed to determine if performance exceeds, meets, or does not meet a given standard and acceptable quality level. The Quality Assurance Inspection Checklist (Enclosure 1) employs a numerical scale using the numbers 1 through 4.

1. Acceptable

2. Unacceptable

3. Work in Progress

4. Work Not Started

9. DOCUMENTING PERFORMANCE

The Government shall document all performance using the attached Quality Assurance Inspection Checklist.

Report may become a part of the supporting documentation for fixed fee payments or other actions. When unacceptable performance occurs, the COR shall inform the contractor. The Government Inspector will document each occurrence on the Inspector Checklist unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file or in the Wide Area Workflow Database.

When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's task manager or on-site representative. A CDR template is attached to this QASP.

The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability.

Any CDRs may become a part of the supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other actions deemed necessary by the KO.
