About this file

This is a Quality Assurance Surveillance Plan (QASP) matrix that outlines performance standards and incentives for a medical waste removal services contract. The matrix establishes specific performance metrics across seven key areas: employee approval, safety requirements, facility services, invoice accuracy, report timeliness, report accuracy, and issue resolution.

The QASP details the quality control measures and financial penalties for non-compliance. Key requirements include using only approved contractor employees (5% invoice deduction for violations), maintaining zero accidents/injuries (1% deduction per incident beyond 2 per year), adhering to pickup/service schedules (2% reduction per incident beyond 1 per month), ensuring accurate invoicing (1% deduction per incorrect invoice beyond 2 per year), timely and accurate reporting (1% deduction per late/inaccurate report), and resolving issues within 24-48 hours (3% deduction for problems unresolved after 7 working days). The plan implements 100% inspection for reporting requirements and periodic inspections for operational performance measures.
