About this file

This is a Quality Assurance Surveillance Plan (QASP) template for the repair or replacement of Generator 4A Switchgear at VA Connecticut Healthcare System's West Haven and Newington Campuses. The underlying contract involves inspection, calibration, and testing services for Underground Storage Tanks (UST) and Above Ground Storage Tanks (AST), including associated system components such as piping, day tanks, Automatic Tank Gauging (ATG) systems, overfill protection, and spill prevention systems to ensure compliance with local, state, and federal regulatory requirements.

The QASP establishes performance standards and surveillance methods to be administered by the Contracting Officer's Representative (COR) and monitored quarterly. Key performance objectives include successful completion of repair or replacement work within schedule (95% timely performance), timely submission of deliverables including quotes, materials, work plans, and compliance certifications (95% compliance), full compliance with NFPA, OSHA, EPA, Massachusetts DEP, and VHA standards (100% compliance), site management standards covering cleanliness, waste removal, proper storage, badge use, and safety (100% compliance), and timely resolution of customer issues (95% resolution rate). The COR will conduct periodic inspections and review submittals using a three-tier rating system (Exceeds Standards, Meets Standards, Does Not Meet Standards). Unacceptable performance triggers a Contract Discrepancy Report (CDR) process requiring corrective action plans. Performance assessments occur quarterly with documentation maintained in the COR file and the Contractor Performance Assessment Reporting System (CPARS).
