About this file

The document is a Performance Assessment Plan and Quality Assurance Surveillance Plan (QASP) for a janitorial services contract for Stanislaus River Parks in Knights Ferry, CA. The QASP establishes performance objectives and standards for the contractor, including 100% submission of required documents at the start of each option year and achieving 95% compliance with janitorial service standards. Inspection methods include random checks, government evaluation reports, and customer complaints, with potential consequences for poor performance ranging from payment deductions (25% of CLIN) to contract termination if performance falls below satisfactory levels.

The plan outlines a detailed evaluation process where the Contracting Officer's Representative (COR) will conduct surveillance after each service, assess performance, and provide opportunities for the contractor to correct deficiencies. The contractor may submit a self-assessment, but this will not be the basis for payment recalculation. The Contracting Officer will make final determinations on payment based on the COR's report, with payment re-calculations to be made within 60 days after the evaluation period or 30 days after receiving an approved invoice.

Performance Assessment Plan:

A. Monitoring Performance. The COR and or the designated representative(s) shall conduct surveillance following each service to ensure satisfactory performance. The COR will assess the Contractor's performance in accordance with the Quality Assurance Surveillance Plan (QASP) and provide the Contractor an opportunity to correct the deficiencies listed prior to the next service day. Deficiencies not corrected prior to the next service day will not be invoiced or paid for by the government and be forwarded in a report to the KO.

B. Contractor Self-Assessment. The Contractor may provide a written self-assessment of its performance to the COR to be considered for inclusion to the report to the KO. Where deficiencies in performance are noted, the Contractor shall describe the actions planned or taken to correct such deficiencies and avoid their recurrence. The self-assessment itself will NOT be the basis for the payment re-calculation determination.

C. COR Recommendation. The COR will consider all evaluations and any other pertinent information, including Contractor self-assessment, and will prepare a report to the KO with findings and recommendations. The Contractor will be provided a copy of the report and the COR’s recommendations.

D. Payment Determination. The KO may meet with the COR to discuss the COR's report. The KO will make a final determination in writing as to the percentage of work successfully completed, and the resulting payment to be made. A copy of the determination will be provided to the Contractor prior to the current month’s invoicing date. All KO decisions regarding payment re-calculations are unilateral decisions made solely at the discretion of the Government.

E. Payment Re-Calculations. Notwithstanding any other clause of this contract, payment re-calculations will be made within the later of 60 days after the end of the evaluation period or 30 days after receipt of an approved invoice.

F. The Quality Assurance Surveillance Plan is one evaluation method the government uses to surveil performance to determine whether the Contractor meets the standards of performance as defined in the PWS. The absence of a QASP for any contract requirement, however, shall not detract from its enforceability or limit the rights or remedies of the government under any other provision of the contract in determining the quality of the Contractor performance.
