About this file

This is a Quality Assurance Surveillance Plan (QASP) for the Boiler Plant Comprehensive Contract at VISN 15, Marion VAMC, Contract Number 36C255. The QASP establishes surveillance methods and performance metrics to ensure contractor compliance with boiler plant operations requirements through the five-year period of performance from February 1, 2026 through January 31, 2031. Surveillance will be conducted using direct observation, periodic inspection, random sampling, performance metrics review, contractor self-reporting, and customer feedback. The plan specifies surveillance frequencies for key performance areas including boiler plant safety device testing (semi-annually), boiler combustion tuning (semi-annually), boiler gage/controls/gas sensor calibration (semi-annually), boiler inspections (annually for deaerator and condensate tank), and deaerator non-destructive testing (every five years).

The QASP establishes performance metrics to evaluate contractor success including completion rates of testing and calibration activities, accuracy and timeliness of test results, compliance with VHA guidelines, documentation completeness and accuracy, emergency response time, report status, and identification of test findings and corrective actions. The Contracting Officer maintains contractual authority and approves corrective actions, while the Contracting Officer's Representative conducts surveillance, documents findings, and communicates with the contractor. The Boiler Plant Supervisor provides technical oversight and supports compliance verification. This surveillance plan ensures contractor adherence to VHA Directive for Boiler Plant Operations and the VHA Boiler and Associated Plant Safety Device Testing Manual throughout contract performance.
