About this file
This is a Quality Assurance Surveillance Plan (QASP) for residential treatment and substance use disorder services at VA Pacific Islands Health Care System. The QASP outlines performance metrics and standards for therapeutic and rehabilitative services provided to veterans.
The plan specifies four main performance requirements: 1) Development of treatment plans within 3 business days of veteran entry with 80% compliance rate, 2) Provision of structured group activities for 20 hours per week minimum at 80% compliance, covering substance treatment, mental health support, medical self-care, case management, spiritual support and familial connection, 3) Individual counseling for 1 hour per week minimum at 80% compliance, and 4) Regular communication with VA staff every 10 days and 5 days prior to discharge at 85% compliance. Additional requirements include submission of written discharge summaries within 3 days and notification of unplanned veteran absences within 3 days at 100% compliance. All metrics are monitored through STAR Staff reporting.
