About this file

This is a Quality Assurance Provisions and Special Inspection Requirements form (AFMC Form 807) for the repair of an Azimuth Drive Motor (Part Number DPM56PF4-K7714, National Stock Number 6105013294767FD). The form specifies that standard inspection per FAR 52.246-2 will be conducted at the contractor's facility (source inspection and acceptance) as the item must meet critical technical and performance requirements. Inspection at destination is not feasible because the RTS-SPO lacks proper test equipment and reopening the item's packaging would necessitate costly repacking with no added value.

The technical authority, Jason L. Francom (415 SCMS/GUMAA, 801-586-2174), determined on 01 JUL 2024 that the item qualifies for source-point inspection and acceptance under all three applicable criteria. A previous source will be used for this acquisition, and no pre-award quality survey is required. The form includes a statement confirming that the requirement does not necessitate the use of Class I ozone-depleting substances. The item manager is Christina McGinnis (801-895-1226), and the equipment specification contact is Jason L. Francom (801-586-2884). Quality provisions cannot be downgraded without technical authority coordination, and the technical authority must be notified if award selection is made to a new source.
