1.0 DESCRIPTION OF SERVICES.

1.1 SCOPE OF WORK. The contractor shall provide all management, tools, supplies, equipment, labor, training, personnel, and transportation necessary to perform Public Adress (PA) support services for Hurlburt Field, FL. Support services include setup, on-site support, and tear down of PA equipment for official events held on Hurlburt Field.

2.0 SERVICES SUMMARY.

2.1 SERVICE SUMMARY (SS). The service requirements are summarized into performance outcomes that related directly to mission essential items. The performance thresholds describe the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success and acceptable (satisfactory) performance. The SS and the contractor’s QCP provide information on contract requirements, the expected level of contractor performance and the expected method of Government validation and confirmation of services provided. Procedures as set forth in FAR 52.212-4(a), Contract Terms and Conditions-Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.2 REQUEST PROCEDURES.

2.2.1 REQUEST PORTAL. The contractor shall provide a means for users on Hurlburt to request support for PAs.

2.2.2 CONTACT INFORMATION. The contractor will have publicly available contact information including Phone Numbers, request Website/Portal, Support Forms for Hurlburt’s authorized users to directly contact them for support.

2.2.3 EQUIPMENT REQUEST. The request will include the needed equipment and support from the table below.

Standard wireless lapel microphone with 500ft range

Multi-channel mixer capable of handling a number of inputs specified by the event
Cabling
All associated cables for equipment.
Generator
Generator capable of powering all associated equipment for a 4-hour period.

2.3 TRACKING. The contractor will maintain tracking of PA requests and setups.

2.3.1 AFTER ACTION. The contractor will provide an after-action report to contracting office for each event service that they provide.

3.0 GOVERNMENT PROVIDED PROPERTY AND SERVICES. The Government will provide podiums and electricity, if available, for performance of services by the contractor for the duration period of this contract. Use of this utility shall be approved and monitored by Government personnel and shall be conserved by the contractor.

4.0 GENERAL INFORMATION.

4.1 CONTRACTOR PERSONNEL.

4.1.1 CONTRACT MANAGER. The contractor shall provide a contract manager who shall be responsible for the performance of the work to ensure it meets the requirements of this contract. The name of this person and an alternate or alternates, who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer within fifteen (15) days of contract award. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

4.1.2 CONTRACTOR EMPLOYEES. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

4.1.2.1 EMPLOYEE TRAINING. All contract employees shall be properly trained and certified to perform the required tasks.

4.1.2.2 EMPLOYEE DRESS REQUIREMENTS. Contractor employees shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. All articles of clothing shall be reasonably clean, in good repair, and shall not display any obscene or objectionable words, phrases, sketches or pictures.

4.1.3 CONTRACTOR/EMPLOYEE BASE PASS AND IDENTIFICATION, SPECIAL CLEARANCES, AND VEHICLE PASSES. The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation. The contractor shall submit the employee listing at the pre-performance conference and the Contracting Officer as changes occur.

4.1.3.1 CONTRACT PREREQUISITE. The contractor shall complete and submit the following form to 1 SOCONS within fifteen (15) business days of contract award in order for employees to gain access to the installation:

4.1.4 THE CONTRACTOR EMPLOYEE LIST. This form shall list every employee requiring base access. Every time employees change; it shall be updated to reflect additions and/or deletions. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare the document(s), attach a cover letter, and forward the Employee List to the Visitor Control Center.

4.1.4.1 CONTRACTOR BADGE. Upon notification that your employee list has been forwarded to the Commercial gate Control Center, the employee shall complete the DHS I-9 form and hand carry it to the Visitor Control Center. The form shall be signed by the employee and the employer or designated representative. If everything is in order, they will receive a contractor badge. If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.

4.1.4.2 CONTRACTOR LISTING MAINTENANCE. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

4.1.4.3 CONTRACTOR BADGE AT COMPLETION/TERMINATION. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.

4.2 SECURITY REQUIREMENTS. The installation access will be coordinated with the local Installation Fire Chief and Security Forces Squadron. Contract personnel will not enter restricted areas without escort from authorized personnel. The contractor shall restrict personnel to only those areas required to performing work.

4.2.1 WEAPONS, FIREARMS AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.

4.2.2 PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

4.3 SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act concerning entry requirements in confined spaces and handling potential hazardous substances. Additionally, all personnel shall possess personal protective/safety equipment.

4.4 REPORTING REQUIREMENTS. Contractor shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources and classified or unclassified defense information.

4.5 SMOKING, EATING AND DRINKING. The contractor shall permit smoking, eating and drinking only in designated areas.

4.6 DRIVING LICENSE/ON-BASE REQUIREMENTS. The Contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.

4.7 HOURS OF OPERATION. All PA support including services performed on Federal Holidays and base down days will be scheduled and addressed on a case by case basis.

4.7.1 FEDERAL HOLIDAYS.

4.8 QUALITY ASSURANCE. The Government shall inspect and evaluate the contractor’s performance to ensure services are received in accordance with the requirements set forth within this contract. Contractors are to provide the COR a Monthly Service Report (MSR) once a month twelve (12) times per year, by 15th calendar day of each month. This is to provide the COR information on services to follow up on to ensure quality of the services.
