PERFORMANCE WORK STATEMENT FOR

OIL & WATER SEPARATORS (OWS), SAND SEDIMENTS AND GREASE TRAPS FOR PETERSON AIR FORCE BASE (PAFB) AND CHEYENNE MOUNTAIN AIR FORCE STATION (CMAFS)

SECTION I DESCRIPTION OF SERVICES

1.1 SCOPE OF WORK. Provide all management, tools, supplies, equipment and labor necessary to inspect, pump, clean and remove the Oil/Water Separators (OWS), Sand Sediments and Grease Traps contents located at Peterson AFB, CO and Cheyenne Mountain Air Force Station, CO as defined in this Performance Work Statement (PWS). Conform to all federal, state, local requirements, federal, state, local governments and commercial standards and (AFOSH 91-501) Occupational Safety and Health Administration (OSHA) standards as well as (AFI 91-203). Work shall be performed in accordance with (IAW) the terms and conditions of this PWS. Estimated requirements are listed in Section V and Section VI, Appendix A, Estimated Workload Data for the Oil & Water Separators, Sand Sediments, and the Grease Traps.

1.2 SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act (OSHA) concerning entry requirements in confined spaces and handling of potentially hazardous substances as it pertains to liquid wastes. The following list of federal regulations pertaining to liquid wastes, which is not all- inclusive and are listed for 21 CES and 721 CES in Section V and VI, respectively Appendix B Publications, Forms and Certifications:

· The Clean Water Act, Title 3, “Standards and Enforcement”, and Title 4, “Permits and Licenses”

· Title 29 CFR Part 1910, “Occupational Safety & Health Standards/Administration-Labor”

· Title 40 CFR Parts 104-149 “Water Programs”

· Title 40 CFR Part 110, “Discharge of Oil”

· Title 40 CFR Part 112, “Oil Pollution Prevention”

· Title 40 CFR Part 401-471, “Effluent Guidelines and Standards”

· Title 40 CFR Subpart F, “Releases from Solid Waste Management Units”

· U.S. DOT Federal Motor Carrier Safety Administration (FMCSA), Commercial Motor Vehicle Safety Act of 1986

· CCR 1007-2, Section 18, WASTE GREASE TRANSPORTERS, FACILITIES, AND PERSONAL USERS OF WASTE GREASE

1.3 SPACE ENTRY TRAINING. All OWSs on Peterson AFB or CMAFS are situated below the ground surface and considered a permit-required confined space. However, all OWS are accessible from ground level, and it should not be necessary for the contractor to break the surface plane to clean and inspect the OWSs except for one as identified in 5.1. In the event that the contractor deems it necessary to break the surface plane and enter the confined space, they shall notify the Contracting Officer’s Representative (COR) to coordinate with the Peterson Fire Department or CMAFS Fire Department. Service must be coordinated to ensure fire department staff availability. Known possible hazards associated with other oil/water separators are:

1.4 TESTING. A composite sample of all OWS contents was analyzed at PAFB. These analytical results are provided in the PWS Attachment A & B. None of the processes that discharge effluent to these OWSs have changed since their last analyzation. An analytical testing of the sediment in the OWS was completed for CMAFS and the results are provided in the PWS Attachment C, D & E. Composite Analytical Testing is required every three (3) years for both PAFB and CMAFS, based off of the most current test result dates in Attachment A through E. Once complete, a copy of the report will be provided to the Contracting Officer Representative (COR).

1.5 SCHEDULING MAINTENANCE. The contractor shall initiate scheduling the annual routine maintenance IAW Appendix A “Estimated Workload Data” and with (COR) in 21 CES/CEOES at (719) 556-4297 or 721 CES/CEOES at (719) 474-3951 prior to servicing any OWS or grease traps listed on Appendix A.

1.6 OWS: The contractor shall pump all oil, oil residue, and other waste from separators IAW Section V, Appendix A schedule. The contractor shall drain separator completely from the separators or pit walls and all associated equipment within the separators. Flush down with water and pump out remaining residue as required by the unique configuration of the separators. See Section 3, paragraph 3.1 for water supply requirements. The contractor shall inspect all parts for problems to include concrete walls and floors for cracks, leaks, broken pipes or other disrepair. If the contractor notices any problems during their inspection, they shall immediately provide the information to the COR prior to the water refill. The contractor shall list, on contractor’s letterhead stationery, the condition of each OWS and any noted deficiencies during each inspection, by location, and submit this to the COR with the manifest. Contractor shall transport all materials from oil/water separators to an approved off-base oil recovery and storage facility. At CMAFS the contractor shall replace the sand/dirt mats on the sand traps in bldg. 300 whenever the grate is removed.

1.7 Grease Traps: The contractor shall pump out all grease and residues from trap IAW Appendix A schedule or on an emergency basis (PAFB only) upon notification by the COR. The contractor shall remove waste build up from trap or pit walls and all associated equipment within the pit. Flush down with water and pump out remaining residue as required by the unique configuration of the grease trap. See Section 3, paragraph 3.1 for water supply requirements. Contractor shall inspect all parts for problems to include concrete walls and floors for cracks, leaks, broken pipes or other disrepair. If the contractor notices any problems during their inspection, they shall immediately provide the information to the COR prior to the water refill. The contractor shall have the COR sign the Uniform Waste Grease Manifest (Form WG-

2) after servicing grease traps prior to leaving installation. The contractor shall list, on contractor’s letterhead stationery, the condition of each Grease Trap and any noted deficiencies during each inspection, by location, and submit this to the COR with the Uniform Waste Grease Manifest (Form WG-2). Residual or recharge water must be higher than lower inlets. Clear obstruction in inlet/outlet pipes to ensure unrestricted flow. Transport all materials from grease traps to an approved off-base storage facility. The use of Personnel Protective Equipment is necessary for removal from the grease traps, Tyvex suits and boot covers, protective eye goggles, hard hat, and gloves must be used while working under building 1000 at CMAFS.

1.8 EMERGENCY SERVICE CALLS (PAFB Only). In the event that emergency service is required, the contractor shall begin service within twenty-four (24) hours after notification by the COR.

1.9 DISPOSAL. All liquids and semi-solid waste removed from Peterson AFB and Cheyenne Mountain AFS shall be disposed of off Air Force property in accordance with all applicable Federal, State and local environmental regulations stated in this PWS, Paragraph 1.1.1.

1.10 LICENSES AND PERMITS. The contractor shall be responsible for obtaining any necessary licenses and permits to transport non-hazardous waste in accordance with the Commercial Vehicle Safety Act of 1986, WASTE GREASE TRANSPORTERS, FACILITIES, AND PERSONAL USERS OF WASTE GREASE, HB 10-1125 and for complying with any federal, state, and municipal laws, codes, and regulations applicable to the performance of work. The contractor shall be responsible for all new and updated laws during the duration of the contract. Contractor shall provide proof of such applicable licenses and permits upon request of the Air Force.

1.11 SPECIAL QUALIFICATIONS. Each Contractor shall have a current/valid Commercial Driver's License (CDL) and current insurance IAW FMCSA.

1.12 RECORDS. The contractor shall maintain sufficiently detailed records to indicate the dates of waste pick-ups, the method of transportation, and the location of the disposal site, registration of disposal site and registration of transport vehicle. The contractor shall provide the final copy of the non-hazardous manifest or commercial bill of lading to the COR within five (5) work days of receipt of the request. If there is Hazardous Waste identified, prior to shipping any portion of the removed sediments off either PAFB or CMAFS as a hazardous waste, notify CES Environmental qualified government representative to sign the manifest. Environmental can be reached at (719) 556-7707 or 556-6100. A copy of the final manifest shall be given to Environmental.

1.13 Invoices shall be submitted in Wide Area Work Flow (WAWF) by facility number, frequency, and amount as listed in Appendix A, for both the OWS’s and Grease traps.

SECTION III GOVERNMENT-FURNISHED PROPERTY

3.1 GOVERNMENT FURNISHED PROPERTY. Contractor shall coordinate with the COR to obtain the water source via the water fill station located behind Building 315 at CMAFS and opposite Building 670 at PAFB or a designated Fire Hydrant able to use the Fire Hydrant; the contractor must have a back flow preventer hose to put the water into their truck. See PWS Attachment C for Water Levels for each OWS and Grease Trap.

SECTION IV GENERAL INFORMATION

4.1 HOURS OF OPERATION.

4.2 NORMAL DUTY HOURS. Contractor shall perform services required under this contract during the following hours: 7:30 a.m. to 4:00 p.m. Monday through Friday.

4.3 HOLIDAYS. Routine repair work shall not be required on the following holidays or any holidays observed in lieu thereof:

4.4 EMERGENCY SERVICES (PAFB only). The contractor shall respond within twenty-four (24) hours after notification by the Contracting Officer or COR. The contractor shall respond to the 24 hour response time to an emergency requirement that may fall on a weekend or holiday.

4.5 QUALITY ASSURANCE. The government shall periodically evaluate the contractor’s performance by appointing Contracting Officer’s Representative (s) CORs and alternate to monitor the performance through intermittent on-site inspections and receipt of complaints from base personnel and to ensure services are received.

4.6 SECURITY REQUIREMENTS. The Contractor and all employees shall comply with all Cheyenne Mountain AFS and Peterson AFB security requirements imposed by the local installation commander at all times while on the premises to include not using cell phones while operating a vehicle on Base. Contractor shall obtain necessary badges(s) required for access to Cheyenne Mountain AFS and Peterson AFB. The information contained in the Badge Request Letter is subject to the Privacy Act of 1974 as follows: AUTHORITY: 5 U.S.C. 4501; 10 U.S.C. 1124; EO 9397; PRINCIPAL PURPOSE: to list the names of personnel authorized to perform various functions under this contract.

4.6.1 ROUTINE USES: To ensure positive identification of personnel authorized to perform the listed functions. The Social Security Number is needed for this identification. Disclosure is voluntary, however, failure to supply any required information may cause denial of entry to or delays in processing access onto Cheyenne Mountain AFS and Peterson AFB. BADGE REQUEST. Contractor shall send a letter to the Contracting Office on company stationery, which shall include address and telephone number. To obtain a badge for those who require access to Peterson AFB or CMAFS, the contractor shall list: the contract number, list full names of employee(s), their social security number, and date of birth. Anytime a badge is requested for a new employee, an updated employee list shall accompany a request letter by the contractor to the contracting office. Contractor employees shall apply for individual identification badges at the Cheyenne Mountain AFS Security Force Squadron Visitor’s Control Center, Bldg 15. Badges are processed Monday through Friday from 7:30 a.m. to 2:30 p.m. Peterson AFB Security Force Squadron Visitor’s Control Center, Bldg 1334. Badges are processed Monday through Friday from 6:00 a.m. to 2:30 p.m. No appointment is necessary. Upon completion or termination of the contract or termination of a contractor employee, the Contractor shall surrender any identification badges and vehicle identification to the Contracting Officer Representative or Security Forces.

4.6.2 ACCESS. Contractor shall apply for flight-line access badge at Base Operations, Bldg. 122, in order to service the flight-line OWS at Peterson AFB.

4.6.3 VEHICLE REQUIREMENTS CMAFS: Proper state vehicle registration, proof of insurance, grease waste registration, and a valid driver’s license are required to drive on base. After obtaining the proper badges/vehicle identification at the Visitor Control Center (Bldg. 15), CMAFS, all commercial vehicles must enter through the Main Gate (Norad Road), the primary search area for all commercial delivery vehicles.

4.6.4 REQUIREMENTS FOR VEHICLE ENTRY INTO CMAFS. Only vehicles used to conduct official business and those affixed with a CMAFS parking decal and an authorization letter from 721 MSG/CC are allowed entry into the restricted area. The Contractor may be permitted to transport all needed personnel, materials, and equipment to the work site. Contractors will load/unload and leave the area. Propane fueled or compressed natural gas vehicles are prohibited within the restricted area. The contractor must submit the request for a vehicle to enter the restricted area on company letterhead; and an officer of the company must sign the letter. The following required information must appear on vehicle request letters: Contract number, model of vehicle, make of vehicle, vehicle license number & state, vehicle identification number, whether gasoline powered and whether the vehicle will be parked temporarily inside the Cheyenne Mountain Complex (CMC). Submit the letter to the COR personnel for processing at least two (2) weeks before requested entry date. The contractor will keep a copy of the vehicle entry authorization letter in each vehicle listed on the letter. The contractor must show the letter to the entry controller each time the vehicle enters the CMC. Because CMAFS is a unique installation, the truck used on the inside of the complex will need to fit through a 9’ - 51/2” wide x 1 0' - 5" high opening.

4.6.5 VEHICLE REQUIREMENTS PAFB: Proper state vehicle registration, proof of insurance, grease waste registration, and a valid driver’s license are required to drive on base. After obtaining the proper badges/vehicle identification at the Visitor Control Center (Bldg. 1334), all commercial vehicles must enter through the East Gate (Marksheffel Road), the primary search area for all commercial delivery vehicles.

4.7 SAFETY ON AN AIR FORCE INSTALLATION: The contractor shall comply with Air Force Instruction (AFI) 91-207 “the US Air Force Traffic Safety Program, (dated 27 Oct 2011), Chapter 3”, is a mandatory policy for each individual while on all Air Force (AF) Installations. All work will be accomplished in accordance with specifications outlined in applicable Occupational Safety and Health Administration (OSHA) and Air Force Occupational Safety and Health Standards (AFOSH).

4.8 PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY. IAW Department of Defense Instruction (DODI) 3020.37, paragraph E2.1.1., a crisis situation is defined as: Any emergency so declared by the National Command Authority or the overseas Combatant Commander, whether or not U.S. Armed Forces are involved, minimally encompassing civil unrest or insurrection, civil war, civil disorder, terrorism, hostilities buildup, wartime conditions, disasters, or international conflict presenting a serious threat to DOD interests. The contractor is not required to perform services during a crisis unless otherwise directed by an authorized government representative.

4.9 GREEN PROCUREMENT AND HAZARDOUS MATERIALS/WASTE. Refer to US Environmental Protection Agency (EPA)’s list of EPA-designed products at www.epa.gov/ Comprehensive Procurement Guideline (CPG) program is part of US Environmental Protection Agency (EPA) continuing effort to promote the use of materials recovered from solid waste. Buying recycled-content products ensures that the materials collected in recycling programs will be used again in the manufacture of new products. Cheyenne Mountain AFS prefers to acquire products that contain recycled-content, bio-based, energy efficient and low volatile organic compounds (VOC). In the accomplishment of this contract, the contractor shall use non-flammable, environmentally friendly, bio-degradable cleaning products. Refer to EPA’s list of EPA-designed products at www.epa.gov/cpg/cpg/products.htm listing 8 CPG product categories, including all the specific products available and provides 8 overview Product Resource Guides and a mechanism to suggest a product.

4.10 HAZARDOUS MATERIALS. In the accomplishment of this contract, the contractor will use non-flammable, environmentally friendly, biodegradable products (IAW) Air Force Instruction (AFI) 32-7086, Hazardous Materials Management. The contractor shall list all proposed hazardous materials to be used on this project on the Air Force (AF) Form 3952, Chemical/Hazardous-Material Request Authorization and Material Safety Data Sheets (MSDS) and submit the AF Form 3952 to the COR, ten (10) days prior to commencing work. The proposed hazardous materials shall be reviewed and authorized for use on this project. Upon request, the contractor must have a copy of the approved MSDS form at the work site. The contractor shall be responsible for containing, handling, and cleanup of all spills, which might occur while providing the maintenance service on all Oil Water Separators listed in this PWS. Cost to dispose of spilled material and any other waste items resulting from the spill will be the responsibility of the contractor. Report immediately all spills to the Base Fire Department at (719) 474-3030 or 911.

4.11 CONTRACTOR MANPOWER REPORTING

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the UNITED STATES AIR FORCE via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/ Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013.
