1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to ensure that commercial hood and duct exhaust cleaning/inspection services are performed at Hill Air Force Base, Utah and Little Mountain Test Facility, Utah in a manner that will ensure continuous and safe operation of such systems and prevent any potential grease fire from spreading past the cook-line into the hood and ductwork. All cleaning shall be in accordance with the most current version of the National Fire Protection Association (NFPA) 96, Chapter 11, (Standard for Ventilation Control and Fire Protection of Commercial Cooking

Operations), Air Force Instruction (AFI) 91-203, para 44.3.6.5, and all other local, state, and federal rules and procedures.

1.1. CLEANING AND INSPECTION SERVICES. The Contractor shall clean and inspect kitchen hood and duct exhaust systems identified in Appendix A in accordance with the most current version of NFPA 96 and all other local, state, and federal rules and procedures. The Contractor shall clean all surfaces on which grease and dust can be deposited in normal operations of each type of equipment to include but not limited to, hoods, grease removal and collection devices, duct systems including horizontal and vertical ductwork from the filters up and throughout the entirety including the fan and blower hosing, fan, fan wheel, exhaust fan and other apparatus thereto. T he Contractor is not responsible for cleaning the motor hoods, canopies or motor interiors. Where there is more than one system in the building, the Contractor shall complete all work required on one system before starting the next. Two or more hoods that vent into one exhaust system going to the roof is considered one system. The Contractor shall inspect and document defects in systems (e.g. broken fan blades, nonfunctioning blowers, frayed fan belts, broken conduit, missing filters, broken hatches, missing hinges, inaccessible areas, separation of ductwork joints, etc.), that, if not corrected, would impair the system’s ability to function properly or impact the Contractor’s ability to perform the service.

1.1.1. HOODS. The Contractor shall clean all surfaces of hoods to bare metal. This includes, but not limited to outer and undersides of hoods and filter compartments. There shall be no visible grease, either solidified or baked on after cleaning is performed.

1.1.2. GREASE REMOVAL AND COLLECTION DEVICES. The Contractor shall clean all grease filters, baffles, collection trays and other approved grease removal and collection devices for use with commercial cooking equipment.

1.1.3. EXHAUST DUCT SYSTEMS. The Contractor shall clean all horizontal and vertical ductwork to bare metal beginning at the hood connection and extending to and including the exit stack. Where ducts are joined in sections, the Contractor may remove and clean if required to ensure adequate cleaning in accordance with this performance work statement and all referenced publications. Duct sections shall be reinstalled with joints opposite to the direction air flow using sheet metal screws and new clips.

1.1.4. BLOWER UNITS. The Contractor shall clean the complete fan assembly including the housing, wheel shafts, braces, supports, pulleys, and all other apparatuses thereto, with the exception of motor interiors.

1.2. CLEANING METHODS. Acceptable methods of cleaning commercial kitchen exhaust systems identified in Appendix A shall include but not limited to the following processes or a combination thereof:

a) Manual removal of cooking by-product

b) Low pressure application of USDA or facility approved detergents

c) Wash/rinse processes utilizing high pressure hot water cleaning

d) Wash/rinse processes utilizing saturated steam cleaning

1.2.1. PROHIBITED PRODUCTS. In accordance with paragraph 11.6.6 of the NFPA 96 Standard, flammable solvents or other flammable cleaning aids shall not be used in cleaning operations.

1.2.2. ELECTRICAL. The Contractor shall, at the start of the cleaning process, lock out any electrical switches that could be activated accidentally during the cleaning process. At the close of cleaning operations exhaust blowers shall be restarted, all access doors utilized during cleaning shall be replaced and fastened, dampers positioned for proper exhaust airflow and electrical switches and system components returned to operable state. When cleaning procedures are complete, all electrical switches shall be returned to an operable state.

1.2.3. ACCESS PANELS AND DOORS. When cleaning procedures are complete, all access panels

(doors) and cover plates shall be restored to their normal operating condition. When access panels are removed, a service company label or tag reprinted with the name of the company annotated with the date of cleaning/inspection shall be affixed near the affected access panel(s).

1.2.4. FIRE SUPPRESSION SYSTEMS. The Contractor shall not render fire suppression system components inoperable during the cleaning process. Cleaning chemicals shall not be applied to fusible links or detection devices of automatic fire suppression systems installed in exhaust equipment.

1.2.5. CLEAN-UP. The Contractor shall keep the service area free from accumulations of waste material or rubbish. When cleaning from the roof down through the ducts, plastic sheeting shall be positioned and affixed directly under hoods to prevent steam from coming into contact with personnel working below. Upon completion of the cleaning service, the Contractor shall ensure the work area and premises are clean and restored to their original condition. The Contractor shall collect and remove all trash, condensed steam, grease and dirt generated from the cleaning to an off-base disposal site. The

Contractor shall provide all cleaning equipment and supplies such as, but not limited to, brooms, mops, buckets, rags, paper towels, etc.

1.3. CLEANING FREQUENCIES AND SCHEDULING. The Contractor shall follow the cleaning frequency schedules below unless otherwise preapproved in writing from the Contracting Officer

(CO). The Contractor shall coordinate directly with the facility manager or point of contact (POC) at each facility on an acceptable date and time to perform the cleaning/inspection that minimizes disruption to the service provided at each facility. The Contractor shall provide the Contracting

Officer Representative (COR) a copy of cleaning schedules via email no later than three (3) days prior to the scheduled cleaning. The Contractor shall provide facility managers or POC’s a minimum of 24 hours notice of any schedule changes. The Contractor shall also notify the COR via email of any schedule changes.

1.3.1. ANNUAL CLEANINGS. Systems in Appendix “A” identified as annual cleanings shall be serviced no later than 365 days and no earlier than 330 days from previous cleaning service.

1.3.2. SEMI-ANNUAL CLEANINGS. Systems in Appendix “A” identified as semi-annual cleanings shall be serviced no later than 180 days and no earlier than 165 days from previous cleaning service.

1.3.3. QUARTERLY CLEANINGS. Systems in Appendix “A” identified as quarterly cleanings shall be serviced no later than 90 days and no earlier than 75 days from previous cleaning service.

1.3.4. BI-MONTHLY CLEANINGS. Systems in Appendix “A” identified as bi-monthly cleanings shall be serviced no later than 60 days and no earlier than 50 days from previous cleaning service.

1.3.5. MONTHLY CLEANINGS. Systems in Appendix “A” identified as monthly cleanings shall be serviced each month and no earlier than 21 days from the previous cleaning service.

1.4. REPORTS AND DOCUMENTATION. The Contractor shall prepare and submit the following reports and documentation as follows:

1.4.1. CLEANING AND INSPECTION REPORTS. The Contractor shall develop and submit to the

Government for approval, a written cleaning and inspection report within ten (10) calendar days after contract award. The Contractor shall submit a copy of each completed cleaning/inspection report to the COR by the tenth calendar day following the month of service. Submittals may be a direct copy of the original or a scanned version delivered electronically in PDF format. The Contractor shall also provide a copy of completed reports to facility managers or the designated point of contact at the time of service. The report shall contain, at a minimum, the following:

a) Building number/name of facility where cleaning was performed

b) Date and time of cleaning

c) Method of cleaning used

d) Acknowledgement certificate of cleaning service label was applied to exhaust hood per para. 1.4.2.

e) Equipment inspection checklist and comment section for identifying system defects.

f) Inaccessible areas that were not cleaned

g) Name and signature of service technician and facility manager/person in charge

1.4.2. SERVICE LABEL. After a cleaning/inspection service has been accomplished for each kitchen exhaust system, a service label 5.5 inches by 4.5 inches in size shall be placed conspicuously, readily accessible, visible, and legible, on the hood in the immediate vicinity of the suppression system tag or existing hood label. The label shall be writeable and made of heat resistant, self-destructing material with self-sticking adhesive on the back. The following shall be included on all service labels:

a) State Fire Marshal’s Seal of Registration

b) Contrasting background, DO NOT REMOVE BY ORDER OF THE STATE FIRE MARSHAL

c) Company name or identifier (logo) with the company’s full address and 24-hour emergency service contract phone number

d) Technician Signature line (shall be signed by lead technician after each service)

e) Certificate of Registration “KEE” number (shall be handwritten by lead technician after each service)

f) This system has been: Inspected or Serviced, line

g) Check box to identify system deficiencies & reference service report

h) All information pertaining to complete date of service: year, month, day

i) All information pertaining to next service due

j) Where applicable, all information above regarding current service and next scheduled service shall be clearly punched using a hand punched into the label after service is complete

1.4.3. SYSTEM DEFECTS. Defects in systems found by the Contractor shall be documented on the cleaning/inspection report at the time of the inspection and reported to the COR within two (2) business days.

2. SERVICE SUMMARY. The contract service requirements in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The Service Summary (SS) and the

Contractor’s Quality Control Plan provide information on contract requirements, the expected level of

Contractor performance. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions – Commercial Items, Inspections/Acceptance, will be used to remedy all deficiencies. During the first initial 30 days of the contract, two (2) additional defects shall be allowed in an effort to identify normal phase-in problems.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government shall provide reasonable quantities of utilities (water and electricity) to the Contractor without charge for the sole purpose of fulfilling the services in this contract.

3.1. UTILITIES. All reasonable quantities of utilities (water and electricity) will be available to the

Contractor without charge. Any temporary lines and connections that may be required shall be approved by the COR and installed, maintained, and removed by the Contractor at the Contractor’s expense.

3.1.1. UTILITY CONSERVATION. The Contractor shall practice utility conservation and perform work in a matter that prevent the waste of such utilities. Water faucets and/or valves shall be turned off after use. Lights shall be used only in areas where and when work is being performed. The Contractor is not authorized to adjust mechanical equipment controls for heating, ventilation, and air conditioning systems unless specially authorized by the person having authority over the system.

3.2. SECURITY, FIRE AND MEDICAL SERVICES. The Government will provide the following emergency services:

3.2.1. SECURITY. The Government will provide general on-base security police service. Security police telephone numbers are 911 for emergencies (When dialing 911, specify you are on Hill Air

Force Base) and 777-3056 for the Law Enforcement Desk

3.2.2. FIRE AND EMERGENCY SERVICES. The Government will provide general on-base fire protection services. Fire extinguishers for facilities are purchased and maintained by the organization occupying the space. The Contractor shall comply with fire prevention rules and regulations enforced by the Hill Air Force Base Fire Prevention Branch. Dial 911 for emergencies and 777-3021 for non-emergency calls. Note: When dialing 911, specify you are on Hill Air Force Base.

3.2.3. EMERGENCY MEDICAL SERVICES. The Government will provide on-site emergency medical treatment for Contractor personnel who are injured or become seriously ill while on Hill Air Force

Base. Ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis. Note: When dialing 911, specify you are on Hill Air Force Base.

4. GENERAL INFORMATION

4.1. QUALITY CONTROL. The Contractor shall develop and maintain a quality program to ensure cleaning and inspection services are performed in accordance with the most current NFPA Standard

96, Air Force Instruction (AFI) 91-203, state and federal rules and procedures and all other commonly accepted commercial practices. The Contractor shall develop and implement procedures to identify and prevent defective services from reoccurring. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in paragraph 2, Service Delivery Summary. The

Government evaluator must have a specific quality control inspector to notify in case of customer complaints.

4.1.1. CONTRACTING OFFICER REPRESENTATIVE (COR). The Contracting Officer Representative

(COR) is the authorized Government representative who will perform assessments of the Contractor’s performance. Subsequent to contract award, the identity of the COR, with a letter defining their duties and authority will be promptly furnished to the successful bidder/offeror.

4.1.2. QUALITY ASSURANCE. The COR and fire inspectors from the Hill Air Force Base Fire

Prevention Office will periodically evaluate the Contractor’s performance. The COR will monitor performance to ensure services are received. The COR will evaluate the Contractor’s performance through periodic on–site inspections of the Contractor's quality control program and receipt of customer complaints. Fire Inspectors who understand the requirements of this contract, will also inspect facility kitchen exhaust systems to ensure they meet facility fire safety codes and will report cleaning defects to the COR. The Government may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate because of repeated failures discovered during quality assurance inspections or because of repeated complaints. Likewise, the Government may decrease the number of quality assurance inspections if merited by good performance. The

Contractor shall be responsible for initially validating customer and/or fire inspector complaints;

however, the COR shall make final determination of the validity of the complaint(s) in cases of disagreement with customer(s). The COR or alternate will inform the CO when discrepancies occur and will request corrective action. The COR or alternate will make a notation of the discrepancy on their surveillance checklist with the date and time the discrepancy was noted and will request the contract manager (or authorized representative) to initial the entry on the checklist. The CO shall follow the requirements of the FAR 52.212–4, Contract Terms and Conditions for Commercial Items

(Feb 2007), for Contractor’s failure to correct nonconforming services.

4.2. FACILITY/SYSTEM CHANGES. As requirements change, the Government may add or drop entire facilities and/or individual kitchen exhaust systems identified in Appendix A. In either case, the

Government shall notify the Contractor in writing, a minimum of ten (10) days of any changes. Any matter concerning a change to the scope, prices, terms, or conditions of this contract shall be referred to the CO and not to the COR.

In the event a facility temporarily closes or has a change in operation where kitchen exhaust systems were not utilized since the last cleaning service, the Government shall notify the Contractor in writing a minimum of ten (10) days prior to the next scheduled cleaning that a cleaning is not required. In which case the Contractor will not perform a cleaning nor invoice the Government for that facility for that scheduled frequency. The services to be performed by the Contractor during the period of this contract shall at all times be subject to review by the CO or authorized representative.

4.3. HOURS OF OPERATION. The Contractor shall perform the majority of services required under this contract during the following hours: 0700 – 1800 Monday through Friday. Due to the nature of business at some facilities, the Contractor may be required to perform services on weekends and/or after 1800 hours.

4.4. RECOGNIZED HOLIDAYS. The Contractor will not be expected to work on the following federal holidays. Additionally, most facilities on the contract will be closed on these holidays. If these holidays fall on a Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed.

4.5. BASE CLOSURES. Work scheduled but not accomplished because of a base closure due to weather, base exercise, or actual alert, will be rescheduled and accomplished as soon as possible after base reopening.

4.6. SECURITY REQUIREMENTS (CONTRACTOR EMPLOYEE BASE ACCESS). The

Contractor shall comply with all requirements and procedures IAW FAR 5352.242-9000, Contractor

Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference. The Contractor shall be required to obtain and display identification badges for all employees requiring access to Hill Air Force Base and Little Mountain Test Facility. These badges will be Government furnished and issued to Contractor employees upon initial entry, under contract, to

Hill Air Force Base and Little Mountain Test Facility. The Contractor shall complete an AFMC 496, Application for AFMC Identification Card for each employee. Requests will be submitted to Pass and

Registration (Building 430) through either the CO or the 75 Civil Engineer Group Security Manager.

Upon approval, the Government will provide and issue a Defense Biometric Identification System

(DBIDS) Credential which shall be worn on the outer clothing between the neck and waist on the front of the body and visible at all times with the Contractor’s name plainly displayed. The DBIDS badge authorizes the Contractor employee access onto Hill Air Force Base and Little Mountain for the sole purpose of fulfilling the requirements of this contact. The DBIDS badge does not authorize the holder access to classified information or unescorted entry into controlled and restricted areas. Loss of the

DBIDS badges shall be reported to the COR or CO not later than the following workday. Identification cards will be surrendered to Security Forces upon employee termination, card expiration or expiration of this contract. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

4.7. SAFETY REQUIREMENTS AND REPORTS. The Contractor is solely responsible for compliance with all federal, state, and local laws, the Occupational, Safety and Health Act (OSHA)

(Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable and shall perform all work in a safe manner as required by these standards. The Contractor shall implement an Occupational Safety and Health Plan to ensure the safety of their personnel, as well as the protection of Government personnel and property. Additionally, the Contractor is responsible for the safety and health of all sub-Contractor employees. The Contractor shall provide a verbal report to the CO within 24 hours of each occurrence an accident resulting in death, injury, occupational disease, or adverse environmental impact. The Contractor shall provide a completed copy of required accident investigation reports to the CO within five calendar days of each occurrence.

4.8. DAMAGE TO GOVERNMENT EQUIPMENT AND PROPERTY. The Contractor shall take all necessary precautions to prevent damage to Government equipment and property. The

Contractor shall report damage of Government property or equipment to the CO and COR with one (1) hour of the incident.

4.9. SPECIAL CERTIFICATIONS AND QUALIFICATIONS. The Contractor shall be licensed and certified by the Office of the Utah State Fire Marshal and have a minimum of three (3) years hands-on experience in commercial kitchen exhaust cleaning service.

4.10. PHASE OUT. In the event there is a change in Contractor or if the operation reverts to in-house operations, the incumbent Contractor shall cooperate to the full extent required to permit an orderly change over and familiarize the follow-on Contractor or Government employee(s) with the kitchen exhaust systems identified in Appendix A.
