PERFORMANCE WORK STATEMENT (PWS)
FOR
DEMO BUILDING 91021
HURLBURT FIELD AIR FORCE BASE, FL.
19 FEB 2025
1. DESCRIPTION OF SERVICE: The Contractor shall provide all labor, equipment, tools, materials, supplies, and other items and services necessary to perform demolition services of building 91021 at Hurlburt Field, Florida as defined in this PWS. All work shall be completed in accordance with (IAW) all federal, state and local laws and regulations.
1.1 GENERAL DESCRIPTION OF PROJECT. The facilities will require a complete demolition and site restoration.
1.2 DEMOLITION. Prior to demolition, the Contractor must submit a FDEP Form 62-257.900(1), “Notice of Demolition or Asbestos Renovation” to the Florida Department of Environmental Protection at least 10 working days in advance, even if there is no asbestos in the structure. The demolition must be performed by a Florida Licensed Demolition or Construction contractor. The Contractor shall perform wet demolition in its entirety. This includes, but is not limited to, all walls, roof, slabs, sidewalks, footings, drain pits, building contents, equipment pads, curbs and asphalt. Hazardous materials shall be disposed of IAW all federal, state and local laws and regulations. All construction and demolition debris (C&D), which are not hazardous materials/waste, to include mechanical equipment, shall be recycled, if possible, at permitted/licensed recovered materials/recycling facility. Disposal weight tickets for C&D debris and recycled material must be provided to 1 SOCES Environmental as part of the project closeout.
1.3 UTILITY WORK. Disconnection and capping of building utilities (water, sewer,) have been completed by Civil Engineer water shop. Contractor is responsible to coordinate with any local utility companies responsible for additional cut-offs. Civil Engineering Electric shop has disconnected electrical service from transformer and Contractor shall remove all remaining electrical cables.
1.4 SITE RESTORATION. The Contractor shall backfill entire area and fine grade grass areas to smooth, even surface with loose, uniformly fine texture. Roll, rake and drag lawn areas, remove ridges and fill depressions, as required to meet finish grade. Limit fine grading to areas which can be planted immediately after grading. Lay sod to form a solid mass with tightly fitted joints. Tightly butt ends and sides of sod strips; do not overlap. Stagger strips to offset joints in adjacent courses. Continuously rake soil in area of installation to assure a smooth finish grade. Tamp or roll lightly to ensure contact with sub-grade. Work sifted soil into minor cracks between pieces of sod; remove excess to avoid smothering of adjacent grass. Sod indicated areas within contract limits and areas adjoining contract limits disturbed because of construction operations. Water newly reconditioned turf areas utilizing water truck or tank as required to establish turf and maintain for 60 days. These areas include but are not limited to foundation, open trenches, asphalt and curb removed areas.
2.0 SUBMITTALS. Make all submittals required by the contract in a timely manner to allow demolition of the building within the allotted performance time. Long lead items such as electrical and mechanical systems required caps, special equipment, etc. must be submitted within 10 days after receipt of the Notice to Proceed.
2.1 ENVIRONMENTAL PROTECTION PLAN. The Contractor shall submit an Environmental Protection Plan within 10 days after receipt of the Notice to Proceed. Approval of the Contractor's plan will not relieve the Contractor of responsibility for adequate and continuing control of pollutants and other environmental protection measures. The Contractor shall obtain approval by the Contracting Officer prior to the start of construction, modification, or demolition for all project facilities and/or equipment. At a minimum the plan shall include the following:
1. Legal Requirements: A list of federal, state, and local laws, regulations, and permits concerning environmental protection.
2. Environmental Protection Procedures: Procedures to be implemented to provide the required environmental protection, to comply with the applicable laws and regulations.
3. Solid Waste Management Plan.
4. Emergency Response and Spill Prevention Plan.
5. Storm Water Pollution Prevention Plan.
6. Hazard Waste Management and Disposal Plan.
2.2 PROJECT SCHEDULE. Contractor shall submit for approval selective demolition schedule within 5 days after receipt of the notice to proceed. Schedule shall include but not limited to; submittals, mobilization, abatement schedule, demolition phase and methods for restoration of site areas schedule.
2.3 AF FORM 103. The base Civil Engineering will assist the Contractor with efforts to fully complete the Base Civil Engineering Work Clearance Request (AF Form 103). It is the responsibility of the Contractor to route the form to each department for signature. It is the Contractor’s responsibility to have a complete approved AF Form 103 prior to beginning any construction activities including clearing, grubbing, exploratory geophysical work, etc.
3.0 GENERAL INFORMATION
3.1 Quality Control: The Contractor shall develop and maintain a quality control program to ensure services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur.
3.2 Quality Assurance. The Contractor shall establish and maintain quality control to assure compliance with the contract, and maintain records of his quality control for materials, equipment and operations. The Government reserves the right to perform inspections as it deems appropriate. The Authorized Government Representative (AGR) will use the Contractor’s project schedule or modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the Contractor’s performance.
3.3 Reporting Procedures. The Contractor shall provide contact information for the project manager assigned. The project manager shall have means of being contacted at all times while on Hurlburt Field.
3.4 Technical Direction. Contact numbers for the designated representative. (Operations Support Section, Civil Engineering) are (850) 884-7711 during normal duty hours. The Service Call Center (850) 884-6683 is the contact point after normal duty hours.
3.5 Emphasis on Government Authorized Representative. Any work performed that is not authorized by the Contracting Officer through the AGR is done at the Contractor’s own risk and responsibility.
3.6 Hours of Operation. The Contractor shall perform the services required under this contract during the base hours of operation: 6:00 A.M. to 6:00 P.M, Monday through Friday. The Contractor may find it necessary to deviate from the normal base hours of operation (i.e. working on Saturday or Sunday) due to weather conditions or unforeseen circumstances, to ensure timely completion of work under this PWS at no additional cost to the Government. Work performed outside of the normal base hours will have to be approved 24 hours in advance by the AGR.
3.7 Federal Holidays.

3.8 Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base. Delays caused by the Government or otherwise covered in this section shall not count against the Contractor.
3.9 Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle Passes.
3.9.1 The Contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or Contractor-furnished Contractor identification badges while visiting or performing work on the installation.
3.9.2 The Contractor shall be required to complete the Contractor Employee Listing (provided at the pre-performance conference) for employees to gain access to the installation.
3.9.2.1 The Contractor Employee List. This form shall list every employee requiring access. Every time employees change; it shall be updated to reflect additions and/or deletions. Identify those individuals who require escort authority by including the word “escort” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations please use building numbers or areas (i.e. Bldg. 90618 or Red Horse, AMMO Area, etc.). Once completed, email the list to the Contracting Officer in EXCEL or WORD format. The Contracting Officer will then prepare and attach a cover letter and forward the Employee List to the Visitor Control Center.
3.9.3 During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
3.9.4 Upon completion or termination of the contract or expiration of the identification passes, the Contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.
3.9.5 Failure to comply with these requirements may result in withholding of final payment.
3.10 NATURAL RESOURCES
3.10.1 ENVIRONMENTAL RESOURCES: The environmental resources within the project boundaries and those affected outside the limits of permanent work under this contract shall be protected during the entire period of this contract. The Contractor shall confine activities to areas defined by the drawings and specifications. Environmental protections shall be as stated in the following subparagraphs:
1. The Contractor shall confine all activities to areas defined by the drawings and specifications. In areas indicated on the drawing or specified to be cleared, the Contractor shall not remove, cut, deface, injure, or destroy land resources including trees, shrubs, vines, grasses, topsoil, and landforms without permission. Trees, shrubs and other vegetation not identified for removal shall be protected against removal, injury, defacing and scarring-no ropes, cables, or guys shall be fastened to or attached to any trees for anchorage unless specifically authorized. Where such emergency use is permitted, the Contractor shall always provide effective protection for land and vegetation resources.
2. The Contractor shall not park vehicles or equipment within the drip line of trees. Prior to trimming or removal of trees, Contractor shall coordinate with Civil Engineering. All merchantable timber that is removed shall be limbed and stacked butt to butt in and out of the way location.
3. Prior to any construction, the Contractor shall mark the areas not to be disturbed under this contract. Isolated areas within the general work area, which are to be saved and protected, shall also be marked or fenced. The Contractor's personnel shall be knowledgeable of the purpose for marking and/or protecting particular objects.
4. All earthworks shall be planned and conducted to minimize the duration of exposure of unprotected soils. Side and back slopes shall be protected as soon as practicable upon completion of rough grading. Earthwork brought to final grade shall be finished as indicated.
5. The Contractor shall construct or install temporary and permanent erosion and sedimentation control features as indicated on the drawings. Berms, dikes, drains, sedimentation basins, grassing, and mulching shall be maintained until permanent drainage and erosion control facilities are completed and operative.
6. The Contractor shall thoroughly clean all construction equipment previously used at other sites before it is brought into the work areas, ensuring that soil residuals are removed and that egg deposits from plant pests are not present; the Contractor shall consult with the USDA jurisdictional office for additional cleaning requirements.
7. Protection of Fish and Wildlife Resources: All species of wildlife are protected on Hurlburt
Field, Florida. Feeding, possessing, capturing, and attempting to capture, kill or otherwise harass wildlife is prohibited.
8. Black Bear and American Alligator: Black bear and American alligator sightings are common on Hurlburt Field. Law protects both animals. Feeding, possession, or harassment of an alligator is a second-degree misdemeanor. The Florida black bear is a state listed threatened species. If a bear is sighted, contact Civil Engineering (884-4651) as soon as possible. Construction sites must be cleared of any food or drink items at the end of each workday. Any items that could attract bears must be carried off the base or placed in a dumpster. Ensure all dumpster doors and lids are securely closed at the end of the day to deter bears from entering and feeding.
3.10.2 CULTURAL RESOURCES
1. Historical, Archaeological, and Cultural Resources: Existing historical, archaeological, and cultural resources within the Contractor's work area will be so designated by the Contracting Officer if any has been identified. The Contractor shall take precautions to preserve all such resources as they existed at the time they were first pointed out. The Contractor shall provide and install protection measures for these resources and be responsible for their preservation during the life of the contract. Protective measures will be provided by 1 SOCES/CEIE after consultation with the State Historic Preservation Officer on a case-by-case basis.
2. Artifacts Discovered During Construction: If during excavation or other construction activities any previously unidentified or unanticipated resources are discovered or found, all activities that may damage or alter such resources shall be suspended. Resources covered by this paragraph include but are not limited to: any human skeletal remains or burials;
artifacts; shell, bone, charcoal, or other deposits; rocks or coral alignments, paving’s, wall, or other constructed features; and any indication of agricultural or other human activities.
Upon such discovery or find, the Contractor shall immediately stop work and notify the Contracting Officer and Civil Engineering.
3.10.3 WATER RESOURCES
1. The Contractor shall keep construction activities under surveillance, management, and control to avoid pollution of surface and ground waters. Toxic or hazardous chemicals shall not be applied to soil or vegetation when such application may cause contamination. Monitoring of water areas affected by construction shall be the Contractor's responsibility.
2. Stormwater: The Contractor shall use proper control and management techniques to ensure stormwater criteria are met IAW federal, state, and local stormwater regulations.
Runoff from the construction site or from storms shall be controlled, retarded, and diverted as indicated on the drawings to protected drainage courses by means of diversion ditches, benches, berms, and by any measures required by area wide plans under the Clean Water Act. Prior to any project that disturbs greater than one acre, the Contractor must complete a Notice of Intent with FDEP and have a Stormwater Pollution Prevention Plan approved by Civil Engineering. A notice of termination must also be filed at the conclusion of the project.
3. Silt screens must be installed prior to start of construction. Silt screens and/or other erosion control devices shall be installed on demolition sites that are in or near wetland areas. Silt screens shall consist of trenched and staked filter fabric and trenched and staked hay bales.
Filter fabric must be toed 8 inches into the soil to avoid sediments that would be transported via water under the screen. Hay bales must be placed end to end on the downstream side of the screen and be trenched and staked firmly into the ground. Chinking is usually required to fill gaps between the bales. Silt screens must be maintained properly. Screens and other control devices must be inspected once a week and after any rainfall event totaling 1/2 inch or more to ensure they are in good repair and functioning properly. In areas that experience high flow rates, extra precautions will be necessary to stabilize screens.
Trenching of haybale barriers is required to adequately control runoff. A series of screens may have to be installed in waters that are especially turbid to properly filter out sediments.
Silt screens will remain in place and properly maintained until the site is properly stabilized with sod or seeding.
4. In some cases where severe erosion results in waters becoming turbid despite control measures, regular turbidity monitoring and documentation shall be necessary. Any such documentation shall be forwarded to Civil Engineering for review.
5. Landscaping: All new landscaping will be watered in accordance with the current stage of the Hurlburt Field Water Conservation Policy. All irrigation work will need to be coordinated with Civil Engineering. The use of potable water for irrigation systems is prohibited.
6. Contractor is not authorized sewage holding tanks on base and must procure a portable toilet service contract. The contract must include the correct removal of sewage and maintenance of the portable toilet.
3.11 ASBESTOS ABATEMENT (if required). The work required by this Section includes furnishing all required labor, equipment, materials, and transportation necessary for the proper and safe removal, handling, and disposal of friable and non-friable asbestos-containing materials (ACM).
Work shall be performed in accordance with applicable Government regulations, as specified in this Section. The asbestos-related work for this Contract includes Demolition, removal, and disposal of ACM and the encapsulation of the surfaces from which ACM was removed. Work areas known to contain or not to contain asbestos will be identified to the Contractor. The Contractor shall notify the Contracting Officer of any other areas suspected to contain asbestos which would be affected by the work, and which was not initially identified.
3.12 CODES, PERMITS, AND STANDARDS
3.12.1 Compliance: The Contractor shall comply with applicable federal, state, and local laws, ordinances, codes, rules, and regulations. All work shall comply with applicable codes and regulations as amended. Before starting the work, the Contractor shall examine these Specifications for compliance with codes and regulations applicable to the Work and shall immediately report any discrepancy to the Contracting Officer.
3.13 SUBMITTALS
3.13.1 Unless noted otherwise, the Contractor shall submit to the Contracting Officer 1 copy of each submittal for review and/or approval.
3.13.2 Pre-work Information: The Contractor shall provide the pre-work submittal items listed herein within 10 days after issuance of the Notice to Proceed. Written approval of all pre-work submittals must be obtained from the Contracting Officer prior to start of any asbestos abatement work specified in this Section.

3.14 TESTING REQUIREMENTS AND RESPONSIBILITIES
3.14.1 Air monitoring will be performed before, during, and after asbestos abatement to document airborne asbestos fiber concentrations. The Contractor shall be responsible for personal air monitoring to determine employee exposure, and the level of respiratory protection required, as well as background, area, and final clearance air monitoring. The following paragraphs identify specific Contractor responsibilities.
3.14.2 Inspections, Monitoring and Clearance Testing:
1. The Contractor, at his expense, shall provide all tests required by specified applicable regulations, codes, and standards and any other tests for his use.
2. From each work area an independent Certified Industrial Hygienist (CIH) or an industrial hygienist technician under direct supervision of a CIH, hired by the abatement Contractor at his expense, shall collect and analyze personal, background, area, and final clearance air monitoring samples. Sampling shall be repeated during each different work activity. Sample collection and analysis shall be performed using the OSHA Reference Method as outlined in 29 CFR 1926.1101, Appendix A.
3.15 DISPOSAL AND TRANSPORTATION OF ASBESTOS-CONTAMINATED WASTE
3.15.1 As the work progresses, to prevent exceeding available storage capacity on site or to prevent stacking of drums, remove sealed and labeled drums or bags of ACM from the work area as required.
3.15.2 Sealed and labeled drums or bags shall be used to transport asbestos- contaminated waste to the landfill. Procedures for hauling and disposal shall comply with 40 CFR Part 61, 49 CFR Part 171 and 172, and other applicable state, regional, and local Government regulations.
3.16 Hazardous Materials Use. The Contractor shall obtain approval from 1 SOCES Environmental Element for any chemicals used during the term of this contract. An inventory of materials will be performed prior to commencement of work. Criteria for identifying hazardous waste are contained in 40 CFR, Part 261, Subpart C.
3.16.1 Spill Response. The Contractor will be briefed on Hurlburt Field spill response procedures at the pre-performance conference. The Contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations. In the event that it is necessary to utilize Government material, equipment, or personnel to clean up a Contractor caused spill, the Contractor shall be required to reimburse the Government for all associated costs.
3.17 Hazardous Material/Waste Management. The Contractor will be briefed on Hurlburt Field Hazardous Material/Waste Management Plan at the pre-performance conference. Coordination with 1 SOCES Environmental Element, HAZWASTE Program Manager, is required before the disposal or transportation of all material/waste generated on Hurlburt Field.
3.17.1 Hazardous Material Handling. The Contractor shall obtain approval from the 1 SOCES Environmental Element prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require Safety Data Sheets (SDS). The Contractor shall provide a copy (hard or electronic) of the SDS for each item to 1 SOCES Environmental Element for review prior to any chemicals being brought onto Hurlburt Field. The Contractor shall maintain a copy of the SDS for each hazardous material line item at the project site.
3.17.2 The Contractor shall not use products that are, or contain, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any hazardous material containing one of these banned substances will not be allowed on base.
3.18 Licenses and Permits. The Contractor shall, without additional expense to the Government, obtain any necessary licenses and permits, and comply with any federal, state, and local laws, codes, and regulations applicable to the PWS. The Contractor shall provide proof of such applicable licenses and permits upon request by the Government.
3.18.1 The Government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
4.0 SAFETY REQUIREMENTS
4.1 The Contractor shall be required to place construction and silt fence around entire construction site.
This will be to create a “clear zone” and to limit any runoff from loose soil.
4.2 The Contractor shall protect Government property to prevent damage during period the property is under the control or in possession of the Contractor.
4.3 Mishaps. In the event of an accident or mishap, the Contractor shall take reasonable and prudent action to establish control of the accident or mishap scene, prevent further damage to persons or property, and preserve evidence until released by the AGR.
4.3.1 The Contractor shall record and report promptly (within 1 hour) to the AGR, all available facts relating to each instance of damage to Government property or injury to either Contractor or Government personnel.
4.3.2 If the Government conducts an investigation of an accident or mishap, the Contractor shall cooperate fully and assist the Government personnel in conducting such investigation until the investigation is completed.
5.0. SCHEDULES
5.1 The Contractor shall submit proposed demolition schedules for approval to the Contracting Officer not later than five days after the issuance of the notice to proceed. The AGR must approve changes to the schedule after coordination has been obtained from the appropriate personnel.
6.0 GOVERNMENT FURNISHED PROPERTY.
6.1 General Information. Except for those items or services specifically stated in this section as Government furnished, the Contractor shall furnish everything needed to perform this contract according to all its terms. The Contractor shall provide all user maintenance for the care and upkeep of all Government furnished services listed in this section.
6.2 Government Furnished Utilities. The Contractor and subcontractors are permitted to use existing utilities available at the site. Use is subject to approval by the Contracting Officer. The 1 Special Operation Civil Engineering Squadron must approve all connections. Connection to existing water mains must utilize a backflow preventer, certified within the 12 months prior to use on base.
6.2.1 Utility Conservation. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.
6.3 Medical Services. In the event of a medical emergency, 911 should be contacted. Ambulance transport responds from the Hurlburt Field Clinic Monday through Friday. On Weekends and Holidays, Okaloosa County ambulance transport will respond. One of the units will respond to the 911 call, and if necessary, transport a service provider employee to one of the local hospitals. The service provider shall reimburse the respective agency for these services.
