About this file
The document is a Foreign Country of Concern Attestation form (PUR 1355) used by governmental entities to verify the ownership and operational status of entities seeking to enter into, renew, or extend a contract that would provide access to personal identifying information. The form requires an officer or representative of the bidding entity to attest that their organization is not owned by, organized under the laws of, or principally based in a Foreign Country of Concern, and that such a foreign government does not hold a controlling interest in the entity.
The attestation is a compliance document that requires the signatory to declare under penalties of perjury that the statements made are true, with spaces provided for the printed name, title, signature, and date of the attesting individual. The form appears to be part of a broader procurement process aimed at protecting sensitive information and ensuring that contracts are not awarded to entities with potential foreign government connections that might pose a security risk.
