About this file
This document contains information about the Supported Decision Making Technical Assistance Program (SDM-TAP) Grant application process run by the California State Council on Developmental Disabilities (SCDD). The grant opportunity is focused on systemic change within one or more professional focus areas, including education, I/DD services, medical services, legal services, and financial services, to encourage and honor the implementation of supported decision making (SDM) by individuals from the aging community, those with mental health issues, and those with intellectual/developmental disabilities (I/DD). The intended outcome is to increase professionals' awareness of and ability to honor SDM, allowing for successful implementation statewide, especially in underserved communities. The grant has a total available funding of $430,000 and covers the project period from September 1, 2024, through December 31, 2025, with the potential for extension beyond that date. Applicants must provide information on their organization's qualifications, collaborative partnerships, project design and implementation, target populations, diversity and disparities, work plans, deliverables, outcomes, evaluation, and more.
