About this file

This document is a Price Appendix FAQ providing instructions for completing pricing submission spreadsheets across three contract vehicle types: BPA (Blanket Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount.

BPA Section: Vendors must fill all yellow cells in the spreadsheet while other cells remain locked. After entering the Package Size in column G for each NDC (National Drug Code), remaining cells auto-populate. If a vendor declines to quote a condition set, the Price per NDC field (column F) highlights red to signal non-participation. Price per Unit (column H) auto-calculates using the formula: Price per NDC divided by Package Size. The template provides tables for 10 NDCs; vendors need only complete tables for their products. Vendors with more than 10 NDCs must contact the Points of Contact listed in the RFQ document for an expanded spreadsheet.

ADP Pricing Sections: Both ADP non-FAMP Discount and ADP WAC Discount sections follow similar submission protocols. Vendors must read Formulary Notes at the top of the spreadsheet to understand the Pharmacy & Therapeutics (P&T) committee's decision-making process. For non-FAMP submissions, vendors enter the first non-FAMP price (column F) and Current Annual Federal Ceiling Price (column H), with subsequent rows auto-populating. A zero percent Additional offered refund (column G) highlights red to indicate non-participation. Total Calculated Refund (columns I and J) auto-calculates using the formula: ((non-FAMP-FCP) + (non-FAMP×Y%)). The WAC Discount section applies to products without FCP, such as diabetic supplies, where vendors submit WAC price and discount percentage, with Total Calculated Retail Refund per Package auto-calculating. All three sections accommodate up to 10 NDCs per template, with vendors requesting additional NDC tables through the RFQ POCs. Vendors with questions should contact the POCs referencing the RFQ document.
