About this file
This is a Price Appendix Frequently Asked Questions (FAQ) document providing guidance for submitting pricing information across three distinct pricing methodologies within a federal contracting context.
The document addresses submission requirements for three pricing structures: BPA (Blanket Purchase Agreement), ADP with non-FAMP discount, and ADP with WAC discount. For all three methodologies, vendors must complete yellow-highlighted cells in provided spreadsheet templates while other cells remain locked. The BPA section requires vendors to enter Package Size (column G) and Price per NDC (column F), with Price per Unit (column H) auto-calculating as Price per NDC divided by Package Size. For ADP non-FAMP pricing, vendors input the first non-FAMP price and Current Annual Federal Ceiling Price, which auto-populate across subsequent rows, plus an Additional offered refund percentage with auto-calculated Total Calculated Refund using the formula ((non-FAMP-FCP) + (non-FAMP*Y%)). For ADP WAC discount pricing, applicable to products without FCP such as diabetic supplies, vendors enter WAC price and WAC discount percentage, with Total Calculated Retail Refund per Package auto-calculating. All three sections highlight cells in red when vendors submit zero values, indicating non-participation in specific formulary categories. The document notes that templates accommodate 10 NDCs by default, with vendors required to contact Points of Contact (POCs) on the RFQ document to request additional NDC tables if submitting more than 10 products. All sections direct vendors to review Formulary Notes at spreadsheet tops for P&T committee decision-making context and to contact POCs regarding questions or concerns.
