About this file
This is a Price Appendix Frequently Asked Questions (FAQ) document that provides instructions for completing pricing submission spreadsheets across three distinct pricing methodologies.
The document covers three pricing submission formats: BPA (Blank Purchase Agreement), ADP with non-FAMP Discount, and ADP with WAC Discount. For the BPA section, vendors must fill yellow cells only with locked remaining cells, enter Package Size (column G) which auto-populates related fields, and enter Price per NDC (column F) with Price per Unit (column H) auto-calculating using the formula: Price per Unit = Price per NDC / Package Size. Red highlighting on the Price per NDC cell indicates non-submission for a formulary category. For the ADP non-FAMP Discount section, vendors input the first non-FAMP price (column F) and Current Annual Federal Ceiling Price (column H), which auto-populate subsequent rows, with a 0.0000% Additional offered refund (column G) highlighting red to indicate non-submission. Total Calculated Refund uses the formula: ((non-FAMP-FCP) + (non-FAMP*Y%)). For the ADP WAC Discount section, WAC pricing applies only to products without FCP (such as diabetic supplies and test strips) or covered drugs, with WAC price (column F) auto-populating subsequent rows and 0.0000% WAC Discount (column G) highlighting red for non-submission. All three sections provide pre-formatted tables for 10 NDCs with instructions to contact Points of Contact (POCs) on the RFQ document for submissions exceeding 10 NDCs. Vendors are instructed to review Formulary Notes at the top of each spreadsheet to understand P&T committee decision-making processes.
