About this file

This is a Price Appendix FAQ document providing instructions for completing pricing spreadsheets across three different pricing models used in a federal pharmaceutical contract opportunity.

The document outlines submission requirements for three distinct pricing structures: BPA (Blank Purchase Agreement), ADP with non-FAMP discounts, and ADP with WAC discounts. For the BPA section, vendors must complete yellow cells in pricing tables, with Package Size entered in column G auto-populating remaining calculations. Price per Unit is auto-calculated by dividing Price per NDC by Package Size. Vendors must submit quotes for all desired NDCs; cells highlight red if no quote is submitted for a condition set, signaling non-participation to the P&T committee. The document provides pre-formatted tables for ten NDCs, requiring vendors to complete only tables for their specific products. For ADP non-FAMP pricing, vendors input the non-FAMP price once in column F, which auto-populates subsequent rows; the Current Annual Federal Ceiling Price is entered in column H and similarly auto-populates. A 0.0000% Additional Offered Refund highlights red to indicate non-submission. For ADP WAC pricing, applicable to products without FCP such as diabetic supplies, vendors enter WAC price in column F and WAC Discount percentage in column G; a 0.0000% discount highlights red for non-participation. All three sections include identical provisions allowing vendors with more than ten NDCs to contact Points of Contact for expanded spreadsheets, and instructing vendors to review Formulary Notes at the top of spreadsheets to understand P&T committee decision-making criteria. Vendors encountering issues during completion should contact the RFQ document POCs for clarification.
