About this file
This is a Price Appendix Frequently Asked Questions document that provides instructions for completing pricing spreadsheets across three distinct pricing models: BPA (Blanket Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount.
For the BPA pricing model, vendors must complete yellow cells in spreadsheets with Package Size (column G) and Price per NDC (column F) for their National Drug Codes. The spreadsheet auto-calculates Price per Unit using the formula (Price per NDC / Package Size). Vendors are provided tables for 10 NDCs but only need to complete tables for their actual products; red highlighting indicates non-submission for a Formulary Category Description. For the ADP non-FAMP Discount model, vendors input the initial non-FAMP price (column F) and Current Annual Federal Ceiling Price (column H), which auto-populate for subsequent rows. A 0.0000% Additional offered refund (column G) triggers red highlighting to indicate non-participation. The spreadsheet auto-calculates Total Calculated Refund using the formula ((non-FAMP-FCP) + (non-FAMP*Y%)). For the ADP WAC Discount model, applicable to products without FCP such as diabetic supplies, vendors input the initial WAC price (column F), which auto-populates for subsequent rows. A 0.0000% WAC Discount (column G) indicates non-submission and triggers red highlighting. The Total Calculated Retail Refund per Package auto-calculates for convenience. All three models require vendors to review Formulary Notes at the spreadsheet top for P&T committee decision-making context. Vendors submitting more than 10 NDCs must contact the RFQ Points of Contact for an expanded spreadsheet, and all questions regarding completion should be directed to the RFQ document's listed POCs.
