About this file

This is a Price Appendix FAQ document providing instructions for completing pricing submission spreadsheets for a federal contract opportunity.

The document contains three sections covering different pricing methodologies: BPA (Blank Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount. For the BPA section, vendors must fill yellow cells only, enter Package Size in column G which auto-populates remaining cells, and provide Price per NDC in column F. Price per Unit (column H) auto-calculates using the formula: Price per NDC divided by Package Size. A red highlight in column F indicates the vendor does not wish to submit a quote for that Formulary Category. The document provides tables for 10 NDCs; vendors needing additional NDC tables must contact the Points of Contact (POCs) on the RFQ document. For the ADP non-FAMP Discount section, vendors input the first non-FAMP price in column F and Current Annual Federal Ceiling Price in column H, with subsequent rows auto-populating. Submission of 0.0000% Additional offered refund (column G) triggers a red highlight indicating non-participation. Total Calculated Refund auto-calculates using the formula: ((non-FAMP-FCP) + (non-FAMP*Y%)). For the ADP WAC Discount section, which applies to products without FCP such as diabetic supplies and test strips, vendors input WAC price in column F with auto-population and provide WAC Discount in column G. A 0.0000% WAC Discount submission highlights red. All sections require vendors to review Formulary Notes at the spreadsheet top and contact POCs with questions or concerns about table completion.
