About this file

This document is a Price Appendix FAQ for a Request for Quote (RFQ) that provides detailed instructions for vendors submitting pricing information for pharmaceutical products. The FAQ covers three primary pricing methodologies: Blanket Purchase Agreement (BPA), Additional Discount Program (ADP) with non-FAMP discount, and ADP with Wholesale Acquisition Cost (WAC) discount. For each methodology, vendors are instructed to fill out yellow cells in provided spreadsheets, with specific guidance including: reading formulary notes, auto-population of certain fields, automatic calculations of pricing metrics, and clear indicators when a vendor chooses not to submit a quote for a specific category. The document instructs vendors to contact the Points of Contact (POCs) on the RFQ document if they need more than 10 NDC (National Drug Code) tables or have questions about completing the pricing template, emphasizing the P&T (Pharmacy and Therapeutics) committee's role in evaluating formulary status.
