About this file
The Price Appendix FAQ document provides detailed instructions for pharmaceutical manufacturers submitting pricing information for the Department of Defense (DoD) Uniform Formulary Blanket Purchase Agreement (BPA) and Additional Discount Program (ADP). The document outlines three distinct pricing approaches: BPA (Basic Purchase Agreement), ADP (non-FAMP Discount), and ADP (WAC Discount), with specific guidance for each method. Key instructions include filling in yellow cells, understanding auto-population features, and using specific calculation methods for pricing and discounts.
The document is specifically designed for the August 2025 P&T Committee meeting, which will review newly approved drugs across various therapeutic categories including corticosteroids, immunomodulators, oncological agents, insulins, and targeted immunomodulatory biologics. Manufacturers are provided with spreadsheet templates for up to 10 National Drug Codes (NDCs), with options to request additional tables if needed. If vendors do not wish to submit a quote for a specific formulary category, they can indicate this by leaving certain cells at 0.0000% or allowing them to highlight red, signaling non-participation to the P&T committee.
